目的:通过观察核因子-κB受体活化因子配体(RANKL)和骨保护素(OPG)在胆脂瘤中的分布和表达水平,探讨两者在胆脂瘤骨质破坏中的作用,以及破骨细胞在其中所起的作用。方法:通过免疫组织化学技术和计算机图像定量分析法,检测RANKL和OPG在27例胆脂瘤和11例外耳道正常皮肤中的表达。结果:RANKL主要表达于胆脂瘤上皮层下的成纤维细胞和泡沫细胞的细胞质中;OPG表达于胆脂瘤的上皮层细胞和上皮层下的炎性细胞细胞质中。RANKL和RANKL/OPG比值在胆脂瘤中的表达量明显高于外耳道正常皮肤,差异有统计学意义(P<0.05);OPG在胆脂瘤中的表达量与外耳道正常皮肤之间差异无统计学意义(P>0.05)。结论:RANKL和RANKL/OPG比值的增高与胆脂瘤的骨质破坏有关,破骨细胞在胆脂瘤的骨质破坏中发挥一定的作用。
OBJECTIVE: To observe the distribution and expression of RANKL and OPG in cholesteatoma and to explore the role of RANKL and osteoprotegerin (OPG) in the bone destruction of cholesteatoma. Osteoclasts play a role in it. Methods: The expression of RANKL and OPG in normal skin of 27 cases of cholesteatoma and 11 cases of external auditory canal were detected by immunohistochemistry and computer image quantitative analysis. RESULTS: RANKL was mainly expressed in the cytoplasm of fibroblasts and foam cells under the epithelial layer of cholesteatoma. OPG was expressed in the epithelial cells of cholesteatoma and in the inflammatory cytoplasm of the epithelium. The expression of RANKL and RANKL / OPG in cholesteatoma was significantly higher than that in normal external auditory meatus (P <0.05). There was no statistical difference between the expression of OPG in cholesteatoma and the normal skin of the external auditory canal Significance (P> 0.05). Conclusion: The increase of RANKL, RANKL / OPG ratio is related to the bone destruction of cholesteatoma, and osteoclasts play a role in the destruction of bone in cholesteatoma.
