上海市358例老年前期及老年人铁营养状况调查

对358例老年前期及老年人进行了铁营养状况的调查,调查方法是询问病史、体检、心电图、肝功能及上消化道钡餐检查及有关铁营养指标检查包括血红蛋白(Hb)、血清铁蛋白(SF)和血液锌原卟啉(ZnPP)。调查结果表明缺铁的患病率,男性为10.2%,女性为7.8%,贫血的患病率男性为4%,女性为7.8%,各患病率男女之间无显著差别(P>0.05)。贫血和缺铁年龄患病率60岁以上组要高于60岁以下组。在剔除缺铁者外246例中观察Hb的正常范围,发现Hb值男女间仍存在性别差异(P<0.01),在剔除贫血者外245例中观察SF和ZnPP的正常范围,发现男女间均无显著差异(P>0.05)。SF值较国内报道50岁以下年龄组为显著增高,并有随年龄增加而逐渐升高趋势。本文查出17例贫血中,11例符合缺铁性贫血诊断,此11例中7例继发于胃大部切除术,4例继发于消化性溃疡伴曾有上消化道出血史。作者认为老年人缺铁和贫血的防治重点应放在胃肠疾病的防治上。 A survey of 358 cases of iron status in the elderly and the elderly was conducted. The investigation method was to ask for medical history, physical examination, electrocardiogram, liver function and barium meal examination of the upper gastrointestinal tract, and related iron nutrition indicators including hemoglobin (Hb) and serum ferritin ( SF) and blood zinc protoporphyrin (ZnPP). The survey results showed that the prevalence of iron deficiency was 10.2% in men and 7.8% in women. The prevalence of anemia was 4% in men and 7.8% in women. There was no significant difference in prevalence between men and women. Difference (P>0.05). The prevalence of anemia and iron deficiency age in the group above 60 years old was higher than that in the group below 60 years old. The normal range of Hb was observed in 246 patients who were absent from iron deficiency. Gender differences were found between males and females (P<0.01). The normal ranges of SF and ZnPP were observed in 245 patients who were excluded from anemia. There was no significant difference between the two groups (P>0.05). The SF value is significantly higher than that reported in the age group below the age of 50 in China, and gradually increases with age. Of the 17 cases of anemia diagnosed in this study, 11 cases were diagnosed with iron deficiency anemia. Seven of these 11 cases were secondary to subtotal gastrectomy and 4 cases were secondary to peptic ulcer with history of upper gastrointestinal bleeding. The authors believe that the prevention and treatment of iron deficiency and anemia in the elderly should focus on prevention and treatment of gastrointestinal diseases.
