Kasai 开创的肝门部肝肠吻合术治疗胆道闭锁 (CBA)已40余年,疗效不断提高,长期存活率亦不断增加,但对本术式目前仍有争议。欧美等国学者多认为 CBA 为一进行性不可逆性疾病,肝肠吻合术难以治愈。作者对术后存活20年以上病例进行随访观察,以期对肝门部肝肠吻合术加以评价。日本东北大学小儿外科在 CBA 行肝门部肝肠吻合术300例中,术后存活20年以上者31例,其中1例于28岁时死于肝功能衰竭,另30例中,男10例,女20例,年龄20~24岁。按病理分型:Ⅰ型10例,
Kasai pioneered hepatic portal anastomosis for the treatment of biliary atresia (CBA) for more than 40 years, with continuous improvement of efficacy and long-term survival rate has also continued to increase, but the operation is still controversial. Many scholars in Europe and the United States believe that CBA is a progressive irreversible disease, which is difficult to cure.
