目的:研究猪肺磷脂注射液联合高频振荡通气对新生儿呼吸窘迫综合征的临床疗效和对血气分析指标的影响。方法:选择2007年7月~2016年1月在我院进行诊治的新生儿呼吸窘迫综合征患儿80例,随机分为两组,对照组给予高频振荡通气治疗,观察组在高频振荡通气基础上加用猪肺磷脂注射液。比较治疗前和治疗后1、6、12、24 h两组患儿的心率、呼吸频率、舒张压和动脉血二氧化碳分压、动脉血氧分压、pH值,并观察两组的疗效和不良反应发生情况。结果:观察组的有效率为92.50%,明显高于对照组的72.50%(P<0.05);治疗后1、6、12、24 h两组患儿的心率、呼吸频率、舒张压均较治疗前有明显变化(P<0.05),且观察组的心率和呼吸频率明显低于对照组(P<0.05),舒张压明显高于对照组(P<0.05);治疗后1、6、12、24 h两组患儿的动脉血二氧化碳分压、动脉血氧分压、pH值均较治疗前有明显变化(P<0.05),且观察组的动脉血二氧化碳分压明显低于对照组(P<0.05),动脉血氧分压和pH值明显高于对照组(P<0.05)。结论:猪肺磷脂注射液联合高频振荡通气能明显改善新生儿呼吸窘迫综合征的血气分析指标,具有较好的疗效和较高的安全性。
Objective: To study the clinical effect of combined with high frequency oscillatory ventilation and pulmonary phlebitis injection on neonatal respiratory distress syndrome and its influence on blood gas analysis index. Methods: A total of 80 neonates with respiratory distress syndrome who were diagnosed and treated in our hospital from July 2007 to January 2016 were randomly divided into two groups. The control group was given high-frequency oscillatory ventilation. The observation group under high-frequency oscillation Ventilation based on the use of pig lung phospholipid injection. The heart rate, respiratory rate, diastolic blood pressure, arterial partial pressure of carbon dioxide, partial pressure of arterial oxygen and pH value were compared between two groups before treatment and at 1, 6, 12 and 24 hours after treatment. Reaction occurred. Results: The effective rate in the observation group was 92.50%, which was significantly higher than that in the control group (72.50%, P <0.05). The heart rate, respiratory rate and diastolic blood pressure of the two groups were significantly higher than those of the control group (P <0.05). The heart rate and respiratory rate in the observation group were significantly lower than those in the control group (P <0.05), and the diastolic pressure was significantly higher than that in the control group (P <0.05) The partial pressure of arterial blood carbon dioxide, the partial pressure of arterial oxygen and the pH value in 24 h group were significantly changed compared with that before treatment (P <0.05), and the partial pressure of arterial carbon dioxide in observation group was significantly lower than that in control group (P <0.05), arterial partial pressure of oxygen and pH value were significantly higher than the control group (P <0.05). CONCLUSION: Porcine pulmonary phospholipid injection combined with high-frequency oscillatory ventilation can significantly improve neonatal respiratory distress syndrome blood gas analysis indicators, with good efficacy and high safety.
