目的研究IV型胶原在先天性肌性斜颈胸锁乳突肌中的增生和表达,探讨肌性斜颈纤维化的发生机制。方法采用Masson三色胶原染色法观察胶原的增生情况,免疫组织化学染色法观察IV型胶原的表达情况。结果IV型胶原在病变组表达明显增加,与对照组比较有显著性差异(P<0.01)。对照组肌束间极少见IV型胶原纤维,病变组肌束间及肌细胞周围均可见大量的胶原纤维增生,肌细胞及肌束排列不规律,肌细胞有不同程度的萎缩。病变组中纤维型组IV型胶原较肌肉型组有更多的表达,两者比较有显著性差异(P<0.01),纤维型和肌肉型组与对照组比较均有显著性差异(P<0.01)。结论先天性肌性斜颈胸锁乳突肌的纤维化程度与IV型胶原增生的量有关。
Objective To study the proliferation and expression of type IV collagen in the sternomastoid muscle of congenital muscular torticollis and to explore the mechanism of muscle torticollis fibrosis. Methods Masson’s trichrome staining method was used to observe the proliferation of collagen. The expression of type IV collagen was observed by immunohistochemical staining. Results The expression of type IV collagen in the lesion group was significantly increased compared with the control group (P <0.01). In the control group, rare type IV collagen fibers were found in the muscle bundles of the control group. A large amount of collagen fibers proliferated around the muscle bundles and muscle cells in the lesion group. The muscle cells and muscle bundles were irregularly arranged and the muscle cells were atrophied to varying degrees. The fibrosis type IV collagen in the lesion group was more expressed than the muscle type group, the difference was significant (P <0.01). The fibrosis type and the muscle type group were significantly different from the control group (P < 0.01). Conclusions The degree of fibrosis in congenital musculus sternocleidomastoid muscle is related to the amount of type IV collagen hyperplasia.
