特发性浮胂是指非由心、肾、肝脏疾病或其他已知原因引起的浮肿。有认为这是由于毛细血管通透性增加,血浆白蛋白漏出,血容量减少,导致血浆肾素活性和醛固酮水平升高所引起。病人有钠水潴留的倾向,特别是站立或接受大量的钠负荷时排泄水钠的能力有不同程度的减退。作者认为大多数特发性浮肿的病人是由于滥用利尿剂所致。连续应用利尿剂后引起血浆肾素活性持续增高和继发性醛固酮增多症。最后导致近球旁器增大,肾上腺亦相继增大。临床上开始时无规律地应用利尿剂,停用时尿钠排泄明显减少,体重增加,出现浮肿,于是再用利尿剂,结果加重了继发性醛固酮增多症。特发性浮肿的病人都是妇女,关心自己的体重与体型。而且往往是医务工作者,容易得到利尿剂。饮食中钠和醣的摄入量有较大波动时容易引起一时性体
