目的通过对县、乡、村三级医疗机构卫生服务质量间的相互影响情况进行分析,为后期门诊和住院服务质量相关政策的调整及干预措施的实施提供基础依据。方法选用结构方程模型对宁夏回族自治区2012年“创新支付制度,提高卫生效益”项目资料进行分析。结果县、乡、村三级医疗机构之间的门诊/住院服务质量之间的相互影响均为正向且有意义(P<0.05),其中在扣除了同一级别医疗机构之间的门诊和住院服务质量的相互影响外,乡村两级的相互影响程度最高。结论不同医疗机构间的门诊、住院服务质量存在相互影响的情况,因此在后续干预措施的制定和实施过程中应当考虑到这种相互作用,以便更好地为居民的卫生保健进行服务。
Objective To analyze the interaction between the quality of health service at county, township and village levels and to provide the basis for the adjustment of the policy of outpatient service and inpatient service quality and the implementation of intervention measures. Methods Structural equation modeling was used to analyze the data of “Innovative Payment System, Improving Sanitation Benefit” in Ningxia Hui Autonomous Region in 2012. Results The outpatient / inpatient service quality between county, township and village medical institutions was both positive and significant (P <0.05). After deducting the outpatient and inpatient between the same level of medical institutions The interaction between the quality of service, the highest level of mutual influence at the village level. Conclusions The quality of outpatient and inpatient services varies from one medical institution to another. Therefore, this interaction should be considered in the formulation and implementation of subsequent interventions in order to better serve the residents’ health care.
