听到第三心音有何临床意义?

第三心音与第四心音均是舒张期的一种额外心音,或称舒张期的一种附加音。二者均归因于心室充盈所致。据相应病变的解剖部位及其血流动力学的改变,这些心音可由左、右心室之一或两侧心室同时产生。起源于右心者,最常于胸骨左缘较低部位或剑突下听到,往往于吸气时出现或变得更响亮。起源于左心者,大多在心尖部听到,并在呼气时心音增强。二者均可随体位改变所引起的暂时性循环动力学改变而改变。如心腔静脉回流暂时增加时,可使之听到或变得更响;而静脉回流减少时.则可消失或变低。第三、四心音的强度改变是心腔内住力、心室充盈或心室残余血量等
