目的探讨循环肾素血管紧张素系统(RAS)和胰岛素抵抗之间的关系。方法40名原发性高血压(EHT)患者和15名肾血管性高血压(RVHT)患者均测量血浆肾素活性(PRA)。测量并计算所有患者的胰岛素(IRI)、葡萄糖耐量试验的120minIRI(IRI120)和IRI总和(ΣIRI)以及胰岛素敏感性指标HOMA-R。结果RVHT患者的PRA比EHT患者高(P<0.05)。EHT患者循环PRA与IRI120和ΣIRI呈独立正相关,而RVHT患者循环PRA与IRI120和ΣIRI无相关关系。结论原发性高血压患者的循环RAS对胰岛素敏感性起调节作用。
Objective To investigate the relationship between circulating renin angiotensin system (RAS) and insulin resistance. Methods Plasma renin activity (PRA) was measured in 40 patients with essential hypertension (EHT) and 15 patients with renovascular hypertension (RVHT). All patients were measured for insulin (IRI), glucose tolerance test 120minIRI (IRI120) and IRI sum (ΣIRI), and insulin sensitivity index HOMA-R. Results PRA in RVHT patients was higher than that in EHT patients (P <0.05). Circulating PRA was independently and positively correlated with IRI120 and ΣIRI in patients with EHT, but there was no correlation between circulating PRA and IRI120 and ΣIRI in patients with RVHT. Conclusion Circulating RAS plays a regulatory role in insulin sensitivity in patients with essential hypertension.
