目的:利用CT和MRI检查诊断10例罕见的左肺动脉分支异常起源(partial anomalous left pulmonary artery,PALPA)患儿,并明确该疾病与气管支气管树间的关系及是否存在气管支气管异常。方法 :回顾分析10例PALPA患儿的影像学和临床资料,探讨该疾病与气管支气管树间的关系,评估患儿气管支气管形态及其他合并的心脏畸形。结果:CT和MRI检查,能清晰显示10例PALPA患儿异常起源的左肺动脉分支走形,2例患儿可见一左肺动脉分支异常起源于右肺动脉,并走行于气管后方、食管前方,形成部分性肺动脉吊带;2例患儿异常起源的左肺动脉分支走行于左主支气管前方;6例走行于左支气管前下方。按气管支气管解剖形态,10例患儿可分为3种,正常气管支气管树形态(n=6);气管支气管树形态正常,但存在右侧气管性支气管(n=3);右肺上叶支气管闭锁、气管桥形成(n=1)。5例患儿行手术治疗,诊断结果与影像学诊断一致。结论:PALPA患者存在气管支气管异常和狭窄的可能性,影像学检查可提高其诊断准确率,并为手术治疗提供有价值的信息。
OBJECTIVE: To diagnose 10 cases of children with partial left ventricular dysfunction (PALPA) by CT and MRI and to determine the relationship between the disease and tracheobronchial tree and whether there is tracheal bronchial abnormality. Methods: The imaging and clinical data of 10 patients with PALPA were retrospectively analyzed. The relationship between the disease and tracheobronchial tree was analyzed. The tracheobronchial morphology and other combined cardiac malformations were evaluated. Results: CT and MRI examination showed that left anomalous branches of left anterior descending artery originated from 10 cases of PALPA. The abnormality of the left pulmonary artery originated from the right pulmonary artery in 2 cases and proceeded to the posterior end of trachea and esophagus. Pulmonary artery sling; 2 cases of abnormal origin of left pulmonary artery branch walk in front of the left main bronchus; 6 cases walk in front of the left bronchus. According to the tracheobronchial anatomy, 10 cases were classified into 3 types, with normal tracheobronchial tree morphology (n = 6); tracheobronchial tree morphology was normal but there was right tracheal bronchi (n = 3); upper right lung Bronchial atresia, tracheal bridge formation (n = 1). 5 cases of children underwent surgical treatment, diagnosis and imaging diagnosis consistent. Conclusion: PALPA patients have the possibility of tracheobronchial abnormalities and stenosis. Imaging examination can improve the diagnostic accuracy and provide valuable information for surgical treatment.
