目的探讨CD1a和CD83标记的肿瘤浸润性树突状细胞(TIDC)在乳腺组织中的表达及其与临床病理特征和预后的关系。方法采用免疫组织化学法检测85例乳腺癌组织中CD1a、CD83的表达情况,探讨其与乳腺癌临床病理特征及预后的关系。结果在85例乳腺癌组织中,77例CD1a阳性,大多数表达CD1a的TIDC分布在肿瘤组织中,肿瘤间质及周边组织中表达较少;44例CD83阳性,表达CD83的TIDC多分布在肿瘤周边及间质组织,呈散点状分布。CD1a、CD83均为阳性表达(双阳性)35例。CD1a、CD83双阳性表达与淋巴结转移、孕激素受体(PR)、中性粒细胞-淋巴细胞比值(NLR)无关(P>0.05),而与雌激素受体(ER)、表皮生长因子受体(HER-2)、TNM分期有关(P<0.05)。CD1a、CD83双阳性表达和非双阳性表达患者的中位无病生存期(DFS)均未达。Cox多因素回归分析显示,CD1a、CD83非双阳性表达是影响DFS的独立因素(HR=2.216,95%CI:1.682~3.431;P<0.05)。结论乳腺癌组织中TIDC的特征性标记物CD83、CD1a表达是影响DFS的独立因素,这可能与肿瘤免疫逃逸机制有关,可以作为乳腺癌预后的参考指标。
Objective To investigate the expression of CD1a and CD83-labeled tumor infiltrating dendritic cells (TIDC) in breast tissues and its relationship with clinicopathological features and prognosis. Methods Immunohistochemistry was used to detect the expression of CD1a and CD83 in 85 cases of breast cancer and to explore its relationship with the clinicopathological features and prognosis of breast cancer. Results In 85 cases of breast cancer, 77 cases of CD1a positive, most of the expression of CD1a TIDC distribution in the tumor tissue, the tumor stroma and surrounding tissue less expression; 44 cases of CD83-positive, CD83-expressing TIDC distribution in the tumor Peripheral and interstitial tissue, scattered dot-like distribution. CD1a, CD83 were positive (double positive) 35 cases. The positive expression of CD1a and CD83 was not associated with lymph node metastasis, PR, neutrophil lymphocyte ratio (NLR) (P> 0.05), but not with estrogen receptor (ER), epidermal growth factor (HER-2), TNM stage (P <0.05). The median disease-free survival (DFS) of both CD1a and CD83 double positive and non double positive patients were not reached. Cox multivariate regression analysis showed that non-double positive CD1a and CD83 expression were independent factors affecting DFS (HR = 2.216, 95% CI: 1.682 ~ 3.431; P <0.05). Conclusion The expression of CD83 and CD1a, which are characteristic markers of TIDC in breast cancer, is an independent factor affecting DFS. This may be related to tumor immune escape mechanism and may serve as a reference index for prognosis of breast cancer.
