目的探讨磁共振扩散张量成像(DTI)技术各向异性分数(FA)、表观扩散系数(ADC)与腰骶丛脊神经根受压程度的关系,评价DTI技术在腰骶丛脊神经根受压程度中的诊断价值。方法选取70例腰椎间盘突出患者行常规MRI及DTI检查,获得L_5神经根Lindblon分级(共4级)及FA值、ADC值,分析Lindblon分型与FA值、ADC值的关系,并进行双侧L_3~S_1神经根扩散张量纤维束成像(DTT)。结果患侧L_5神经根平均FA值显著低于健侧(t=-4.416,P<0.01),平均ADC值高于健侧(t=3.334,P<0.05);各级神经根平均FA值、ADC值的差异有高度统计学意义(F=50.347,P<0.01;F=53.572,P<0.01);经Spearman相关分析显示神经根受压Lindblon分级与FA值呈负相关(rs=0.970,P<0.01),与ADC值呈正相关(r_s=-0.963,P<0.01)。结论 FA值、ADC值可以反映神经根受压程度,且随着受压程度的加重,FA值降低,ADC值升高。
Objective To investigate the relationship between the anisotropy fraction (FA), apparent diffusion coefficient (ADC) and the degree of lumbosacral plexus root compression in magnetic resonance diffusion tensor imaging (DTI) technique. To evaluate the effect of DTI technique on lumbosacral plexus root compression The diagnostic value of the degree. Methods 70 patients with lumbar disc herniation underwent routine MRI and DTI examinations. The Lindblon grade of L_5 nerve root (total 4 grades), FA value and ADC value were obtained. The relationship between Lindblon classification and FA value and ADC value was analyzed. L_3 ~ S_1 nerve root diffusion tensor fiber bundle imaging (DTT). Results The average FA value of the injured nerve root in the affected side was significantly lower than that in the healthy side (t = -4.416, P <0.01), and the average ADC value was higher than that in the healthy side (t = 3.334, P <0.05) There was a significant difference in ADC value between the two groups (F = 50.347, P <0.01; F = 53.572, P <0.01). Spearman correlation analysis showed that the Lindblon score of nerve root compression was negatively correlated with FA (rs = 0.970, P <0.01), and positively correlated with ADC value (r_s = -0.963, P <0.01). Conclusions FA value and ADC value can reflect the compression degree of nerve root. With the increase of compression degree, FA value decreases and ADC value increases.
