[目的]了解高血压患者药物治疗依从性现状,探讨高血压患者社会支持和药物治疗依从性的关系。[方法]以横断面调查中的简单随机抽样方法选取上海某区符合条件的调查对象1068名,以调查对象自填问卷为主的方式进行,采用MORISKY推荐的高血压患者药物治疗依从性问卷、社会支持量表对患者的药物治疗依从性、社会支持情况进行调查。[结果]调查对象中70.4%药物治疗依从性佳;正在服药的高血压患者社会支持量表的社会支持总得分为(39.63±7.17)分,主观支持、客观支持和支持利用度的得分分别为:(24.25±4.73)分、(8.49±2.57)分、(6.91±2.21)分;Spearman秩相关分析显示,支持利用度与药物治疗依从性呈正相关,rs=0.85,P=0.005;多因素非条件logistic回归分析显示,支持利用度高、高血压病程长、家庭人均月收入高是药物治疗依从性的促进因素,OR及95%CI分别为:1.57(1.178-2.086)、2.01(1.415-2.866)、1.76(1.223-2.538)。[结论]高血压患者社会支持利用度高者药物治疗依从性佳,构建有效的社会支持系统,提高患者社会支持利用度,并着重对高血压病程短和家庭人均月收入低者进行干预,对提高患者的药物治疗依从性更有意义。
[Objective] To understand the status of drug treatment compliance in hypertensive patients and to explore the relationship between social support and drug treatment compliance in hypertensive patients. [Methods] A total of 1068 eligible patients in a certain district in Shanghai were selected by simple random sampling method in cross-sectional survey. The subjects were mainly self-administered questionnaires. MORISKY recommended medication adherence questionnaire for hypertensive patients, The social support questionnaire surveyed the patient’s medication adherence and social support. [Results] 70.4% of the surveyed patients had good drug treatment compliance. The total social support scores of the social support questionnaire for the hypertensive patients being taken were (39.63 ± 7.17) points, and the scores of subjective support, objective support and supportive utilization were : (24.25 ± 4.73) points, (8.49 ± 2.57) points and (6.91 ± 2.21) points respectively. Spearman rank correlation analysis showed that there was a positive correlation between supportive utilization and medication adherence, rs = 0.85, P = 0.005; Conditional logistic regression analysis showed that the supportive utilization rate, long duration of hypertension and high per capita monthly household income were the contributing factors of drug treatment compliance. The OR and 95% CI were 1.57 (1.178-2.086) and 2.01 (1.415-2.866 ), 1.76 (1.223-2.538). [Conclusion] Hypertensive patients with high utilization rate of social support have good drug treatment compliance, build effective social support system, improve the utilization of social support, and focus on the short duration of hypertension and lower per capita monthly household income, Increasing the patient’s medication adherence makes more sense.
