目的研究胃癌(gastriccancer,GC)形成中核因子-κB(nuclearfactorκappaB,NF-κB)的表达规律与其细胞凋亡、增殖的关系,探讨NF-κB在GC形成中的可能作用机制。方法采用免疫组化染色法(SABC法)和原位末端标记法(TUNEL)检测15例正常胃黏膜(normalgastricmucosa,NGM)、30例胃黏膜肠上皮化生(intestinalmetaplasia,IM)、30例异型增生(Dysplasia,Dys)和40例GC组织中NF-κBp65、增殖细胞核抗原(proliferatingcellnuclearantigen,PCNA)阳性表达和细胞凋亡情况。结果在NGM→IM→Dys→GC的形成过程中,NF-κBp65的表达呈递增趋势,其阳性表达GC组(62.50%)和NGM组(6.70%),GC组和IM组(20.00%)差别均有显著性(P<0.05)。GC组细胞凋亡指数(apoptoticindex,AI)显著低于其他各组(P<0.01)。细胞增殖指数(proliferationindex,PI)则呈递增趋势,GC组与各组比较差异有显著性(P<0.01)。在不同程度癌前病变中NF-κBp65的表达差异无显著性(P>0.05)。从NGM→IM,NF-κBp65与AI呈正相关(r为0.52,P<0.05);从Dys→GC,与AI负相关(r为-0.49,P<0.05)。在GC形成整个过程中,NF-κBp65与PI呈正相关(r为0.57)。结论在GC形成过程中,NF-κBp65的表达呈递增趋势;NF-κB的表达上调可能是GC形成的早期事件;NF-κB在GC形成的不同阶段均有促进细胞增殖作用,但对细胞凋亡的作用不同,早期促进细胞凋亡,后期抑制细胞凋亡。
Objective To investigate the relationship between the expression of nuclear factor-κB (NF-κB) and the apoptosis and proliferation of gastric cancer (GC), and to explore the possible mechanism of NF-κB in GC formation. Methods 15 cases of normal gastric mucosa (NGM), 30 cases of intestinal metaplasia (intestinalmetaplasia, IM) and 30 cases of dysplasia were detected by immunohistochemical staining (SABC method) and in situ end labeling (TUNEL). (Dysplasia, Dys) and 40 cases of GC tissue NF-κB p65, proliferating cell nuclear antigen (PCNA) positive expression and apoptosis. Results During the formation of NGM → IM → Dys → GC, the expression of NF-κB p65 showed an increasing trend, positive expression in GC group (62.50%) and NGM group (6.70%), GC group and IM group (20.00%) difference All significant (P<0.05). The apoptotic index (AI) in GC group was significantly lower than that in other groups (P<0.01). Proliferation index (PI) showed an increasing trend, and there was significant difference between GC group and each group (P<0.01). There was no significant difference in the expression of NF-κBp65 in different precancerous lesions (P>0.05). From NGM → IM, NF-κB p65 was positively correlated with AI (r = 0.52, P <0.05); from Dys → GC, negatively correlated with AI (r = -0.49, P <0.05). During the entire process of GC formation, NF-κB p65 was positively correlated with PI (r was 0.57). Conclusions During the formation of GC, the expression of NF-κB p65 is increasing. The up-regulation of NF-κB may be an early event of GC formation. NF-κB promotes cell proliferation at different stages of GC formation, but it has a negative effect on cells. The role of death is different, early promotion of apoptosis, inhibition of apoptosis in the late.
