目的探讨三峡水库运行后长江江苏段江滩钉螺孳生环境变化对血吸虫病传播的影响。方法收集三峡水库运行前后长江江苏段水情资料、江苏省沿江地区江滩滩情资料及血吸虫病病情和螺情资料,并结合典型滩块现场钉螺调查进行综合分析。结果三峡水库蓄水后,长江江苏段水位呈逐年下降趋势。与建坝前(2002年)相比,三峡水库蓄水175 m后,1-3月份水位提高0.41 m,4-12月份水位降低0.32 m。在水库建成逐渐运行期间,江苏省沿江地区血吸虫病疫情呈下降趋势,2012年全省沿江地区感染性钉螺面积降为0,人群血清学检测阳性率为0.73%,病原学检测阳性率为0.004%。结论三峡水库运行后,长江下游江苏省沿江地区血吸虫病病情和螺情均呈下降趋势。
Objective To explore the influence of the environment of snail breeding on the spread of schistosomiasis in the Yangtze River in Jiangsu Province after the Three Gorges Reservoir operation. Methods Before and after the operation of the Three Gorges Reservoir, the hydrological data of the Jiangsu section of the Yangtze River in Jiangsu Province, the data of riverside beach conditions and the schistosomiasis prevalence and snail information in the Yangtze River in Jiangsu Province were collected and analyzed in combination with the investigation of snails on the typical beach block. Results After the Three Gorges Reservoir was impounded, the water level in the Jiangsu section of the Yangtze River showed a downward trend year by year. Compared with that before the dam construction (2002), the water level of the Three Gorges Reservoir increased by 0.41 m in January-March and the water level decreased by 0.32 m in April-December after the impoundment of 175 m. During the gradual operation of the reservoir, the epidemic rate of schistosomiasis in the riverside area of Jiangsu Province showed a downward trend. In 2012, the area of infectious snails in the riverside areas in the province dropped to 0, and the positive rate of serological detection in the population was 0.73% and the positive rate of etiological detection was 0.004% . Conclusion After the operation of the Three Gorges Reservoir, the prevalence and snail status of schistosomiasis in the Yangtze River lower reaches of the Yangtze River in Jiangsu Province showed a downward trend.
