目的:探讨慢性阻塞性肺病(COPD)患者运动负荷试验中监护的重要性。方法:缓解期COPD患者62例,正常组10例。采用运动心肺功能仪(Sensormedics2900美国)作运动负荷试验,观察患者运动时心率(HR)、呼吸率(RR)、氧饱和度(SO2)变化。结果:COPD组运动时最大心率(HRmax)低于预计值,SO2随运动功率递增而下降,最大功率(Wmax)、运动时间(T)明显低于正常。结论:COPD患者宜采用次极量运动试验,按斜坡法递增方案,运动中要严密监护HR、SaO2变化,以提高COPD患者运动试验的安全性。
Objective: To investigate the importance of monitoring during exercise stress test in patients with chronic obstructive pulmonary disease (COPD). Methods: 62 patients with COPD in remission and 10 patients in normal group. The heart rate (HR), respiration rate (RR), and oxygen saturation (SO2) changes were observed during the exercise stress test using a sports cardiopulmonary apparatus (Sensormedics 2900 USA). Results: The HRmax of exercise in COPD group was lower than the expected value. SO2 decreased with the increase of exercise power. The maximum power (Wmax) and exercise time (T) were significantly lower than normal. Conclusion: Submaximal exercise test should be used in patients with COPD. The incremental method according to the slope method should be adopted. During the exercise, HR and SaO2 changes should be closely monitored to improve the safety of exercise test in COPD patients.
