用“新标准”对某冶金建筑公司98例矽肺胸片进行读片和诊断分期,并与“旧标准”的诊断结果进行比较分析。结果表明两种诊断标准的符合率为83.67%,其中Ⅰ期为94.11%,Ⅱ期为58.62%,Ⅲ期为100%。“新标”较“旧标”少诊断3例矽肺,而从Ⅰ期升Ⅱ期1例,Ⅱ期降为Ⅰ期11例。由于“新标”明确了矽肺分期应把小阴影的类型、密集度和范围结合起来综合评定,使许多在“旧标”中的观察对象的胸片达到Ⅰ期标准,减少了盲目性和片面性。同时由于冶金建筑矽肺发病年龄大,工龄长,工种复杂,突击性大,胸片上以类圆型小阴影为主,且多出现在双上肺野,在诊断时应予以重视。
The “new standard” was used to read and diagnose the stage of silicosis in 98 cases of a metallurgical construction company, and to compare with the diagnostic result of “old standard”. The results showed that the coincidence rate of the two diagnostic criteria was 83.67%, of which 94.11% in stage Ⅰ, 58.62% in stage Ⅱ and 100% in stage Ⅲ. The “new standard” than the “old standard” less diagnosis of 3 cases of silicosis, and from the Ⅰ stage Ⅱ stage in 1 case, stage Ⅱ reduced to stage Ⅰ 11 cases. Because the “new standard” made it clear that the classification, concentration and range of small shadows should be comprehensively evaluated in the silicosis staging so that many of the observed objects in the “old standard” achieve the stage I standard of the chest radiograph, reducing blindness and one-sidedness . At the same time as the age of silicosis of metallurgical construction, long service life, the type of work complex, large assault, the chest-like round-shaped small shadow on the main, and more in the double lung field, should be taken seriously in the diagnosis.
