目的通过经胸超声及经食管超声,评估心房颤动患者的左心耳功能,探索其左心耳自发显影(SEC)及左心耳血栓(LAAT)形成的独立危险因素。方法 (1)将受检查者分为三组。记录三组受检者的年龄及超声数据,对三组数据进行均数比较。(2)根据TEE的结果分为SEC和(或)LAAT组和未见异常组,将两组数据进行多因素Logistic回归分析,探索SEC和(或)LAAT的独立危险因素。结果 (1)三组参数中,年龄及代表左心及左心耳内径的参数随着对照组、A、B组的趋势逐渐增大,代表左心及左心耳功能的参数则逐渐减小。(2)多因素Logistic回归分析显示LAAEF和LAA-PFV为SEC和(或)LAAT的独立危险因素(P<0.05)。结论经食管超声在评估左心耳功能及诊断左心耳血栓方面可提供敏感指标,其中LAAEF、LAA-PFV是SEC和(或)LAAT形成的最强独立预测因素。
Objective To evaluate the left atrial appendage function in patients with atrial fibrillation by transthoracic echocardiography and transesophageal echocardiography, and to explore the independent risk factors of spontaneous development of the left atrial appendage (SEC) and left atrial appendage thrombus (LAAT). Method (1) The subjects were divided into three groups. The age and ultrasound data of three groups of subjects were recorded, and the average of three groups of data was compared. (2) According to the results of TEE, the patients were divided into SEC and / or LAAT group and no abnormal group. Multivariate Logistic regression analysis was used to explore the independent risk factors of SEC and / or LAAT. Results (1) Among the three groups of parameters, the age and the parameters representing the left atrium and the left atrial appendage increased with the increase of the control group. The parameters representing the left atrium and left atrial appendage gradually decreased. (2) Multivariate Logistic regression analysis showed that LAAEF and LAA-PFV were independent risk factors for SEC and / or LAAT (P <0.05). Conclusions The transesophageal echocardiography can provide sensitive indicators for assessing left atrial appendage function and diagnosing left atrial appendage thrombus. LAAEF and LAA-PFV are the strongest independent predictors of SEC and / or LAAT formation.
