[目的]研究血清总胆汁酸(TBA)在婴幼儿肝炎综合征(IHS)中的表达及其临床意义。[方法]采用全自动生化分析技术,测定婴儿IHS组84例与非肝胆疾病患儿(对照组)32例的TBA水平,同时对其肝功能相关项目进行测定,并进行比较分析。IHS组分为肝炎型48例、瘀胆型36例。[结果]①IHS组的TBA表达明显高于对照组(P<0.01);②IHS组瘀胆型血清TBA、总胆红素(TB)、直接胆红素(DB)、丙氨酸氨基转移酶(ALT)、r-谷氨酰转肽酶(r-GT)均高于肝炎型(P<0.01);③IHS患儿TBA与血清TB、DB、ALT、r-GT呈正相关(P<0.01)。[结论]IHS患儿均存在不同程度胆汁瘀积,瘀胆型IHS胆汁瘀积程度更严重。血清TBA的表达在反映肝功能损伤方面是一个敏感的指标,对婴幼儿IHS的早期诊断、治疗有重要临床意义。
[Objective] To study the expression of serum total bile acid (TBA) in infantile hepatitis syndrome (IHS) and its clinical significance. [Methods] The levels of TBA in 84 infants with IHS and 32 infants with non-hepatobiliary diseases (control group) were determined by automatic biochemical analysis. Meanwhile, their liver function related items were measured and compared. IHS components were hepatitis type 48 cases, stasis type 36 cases. [Results] The TBA expression in IHS group was significantly higher than that in control group (P <0.01). ② The levels of TBA, total bilirubin (TB), direct bilirubin (DB), alanine aminotransferase ALT) and r-glutamyl transpeptidase (r-GT) were significantly higher than those in hepatitis (P <0.01). ③TBA in serum of IHS children was positively correlated with serum TB, DB, ALT and r-GT (P <0.01). [Conclusion] There are different degrees of cholestasis in children with IHS, and the degree of cholestasis of IHS cholestasis is more serious in IHS children. Serum TBA expression is a sensitive index in reflecting the impairment of liver function. It has important clinical significance for the early diagnosis and treatment of IHS in infants.
