目的 研究PR间期延长与糖尿病视网膜病变(DR)的相关性,为临床早期发现DR提供参考依据.方法 选取2014年5月至2016年3月于西京医院内分泌科住院的2型糖尿病(T2DM)患者797例为研究对象.用免散瞳的眼底摄像技术对DR进行诊断,将研究对象分为无视网膜病变(NDR)组539例、有视网膜病变(DR)组258例.进行心电图检测得到PR间期,测量血压、血糖和血脂等.用SPSS 19.0软件进行t检验、非参数秩和检验.PR间期、DR与各指标的相关性用Spearman相关分析,PR间期、DR的影响因素分析采用logistic回归分析.结果 T2DM患者的PR间期均值为(162.90±22.25)ms,其中DR组患者PR间期的平均长度为(170.08±23.78) ms,显著高于NDR组[(157.11±19.08) ms],差异有统计学意义(P<0.01).PR间期与DR呈正相关,有统计学意义(r=-0.293,P<0.01).多因素logistic回归分析结果显示,PR>163 ms的T2DM患者是PR≤163 ms的患者患DR风险的2.071倍(OR=2.071,95%CI:1.477~2.905,P<0.01);PR>200ms的T2DM患者是PR≤200 ms的患者患DR风险的4.060倍(OR=4.060,95%CI:1.653~9.972,P<0.01).结论 心电图PR间期与糖尿病视网膜病变具有相关性,应提醒PR间期显著延长的患者进行DR筛查.“,”Objective To explore the correlation between PR interval and diabetic retinopathy (DR),and to provide the basis for clinical finding early DR.Methods the subjects were 797 inpatients with T2DM selected from Xi-jing hospital during May of 2014 to March of 2016.The subjects were divided into non-diabetic retinopathy (NDR,539 cases) group and DR (258 cases) group,according to non mydriatic fundus photography.ECG,blood pressure,blood glucose and blood lipid were measured for all subjects.The t test and nonparametric rank sum test were used to analyze the data with SPSS 19.0.Spearman correlation analysis method was used to analyze the correlation between PR interval or DR and other indexes,the logistic regression analysis was performed for influencing factors on PR interval or DR.Results Mean value of PR interval in all patients was 162.90±22.25 ms.The mean PR interval of DR group was (170.08±23.78) ms,which was significantly higher than that (157.1 1 ±19.08) ms of NDR group (P<0.01).There was positive correlation between PR interval and DR (r=0.293,P<0.01).Multivariable logistic regression showed that the DR risk of T2DM cases with PR>163 ms was 2.071 folds of DR risk of T2DM cases with PR≤ 163 ms (OR=2.071,95%CI:1.477-2.905,P<0.01);the DR risk of T2DM cases with PR>200 ms was 4.060 folds of DR risk of T2DM cases with PR ≤ 200 ms (OR=4.060,95%CI:1.653-9.972,P<0.01).Conclusion ECG PR interval correlate with the diabetic retinopathy (DR) in T2DM patients.DR screen should be taken in T2DM patients with PR interval prolongation.
