15例恶性肿瘤患者在进行骨髓移植(异基因骨髓移植3例、自体骨髓移植6例和混合骨髓移植6例)时应用分次全身照射技术。分次照射方案:8~15MVX线总剂量800~1000cGy,每天2次,共计2天,每次照射间隔时间为6h;剂量率5.98~6.32cGy/min,平均剂量率为6.6cGy/min;肺遮挡后平均剂量为600cGy,胸壁补充9MeVβ线200cGy。随防结果∶3例移植失败(异基因骨髓移植1例、混合骨髓移植2例),1例慢粒急变行自体骨髓移植后10个月因肿瘤复发而死亡,其余11例均无瘤生存;无1例出现放射性损伤。结果提示采用分次照射技术可以提高疗效、减少并发症的发生。
Fifteen patients with malignant tumor underwent whole body irradiation with bone marrow transplantation (3 for allogeneic bone marrow transplantation, 6 for autologous bone marrow transplantation and 6 for mixed bone marrow transplantation). Divided irradiation program: 8 ~ 15MVX total dose of 800 ~ 1000cGy, 2 times a day for a total of 2 days, each irradiation interval of 6h; dose rate of 5.98 ~ 6.32cGy / min, the average dose rate of 6.6cGy / Min; the average dose after lung occlusion was 600 cGy, and the chest wall was supplemented with 200 cGy of 9 MeV β line. With the prevention of results: 3 cases of transplantation failed (allogeneic bone marrow transplantation in 1 case, mixed bone marrow transplantation in 2 cases), 1 case of chronic granulomatous emergency autologous bone marrow transplantation 10 months after the tumor recurrence and the remaining 11 cases were tumor-free survival; No case of radioactive injury. The results suggest that using fractionated irradiation can improve the efficacy and reduce the incidence of complications.
