1986年至1991年我院在门诊用液氮低温冷冻法治疗肛门瘙痒症320例,效果满意。 资料与治疗方法 男228例,女92例,年龄22～65岁,病程1月～12年。经2～5次治疗,治愈250例,好转48例,无效22例,总有效率93％。疗效标准:治愈:症状消失,皮损消除,1年内无复发;好转:症状皮损基本消除,偶有痒感,可用一般治疗缓解;无效:治疗后短期症状缓解,但常有复发。 患者侧卧,充分暴露肛周皮肤,用洁尔灭清洗皮损区。用枪式液氮低温治疗器,按动冷冻开关,使液氮呈细珠状均匀喷射在病变部位,直至表皮形成一层薄霜。治疗时局部有烧灼性痛感,可以忍受,一般不需麻醉,如患者痛觉敏感不能忍受时,可用0.5％利多卡因或普鲁卡因局麻后再施治疗。 讨论:冷冻疗法是利用浓氮超低温(-19℃)
From 1986 to 1991, 320 cases of anus pruritus were treated with liquid nitrogen cryotherapy in our hospital, with satisfactory results. After 2 to 5 treatments, 250 cases were cured, 48 cases improved, ineffective in 22 cases, the total efficiency of 93%. Efficacy criteria: cure: the symptoms disappear, eliminate skin lesions, no recurrence within 1 year; improvement: the basic elimination of symptoms of skin lesions, occasional itching, available general treatment to alleviate; invalid: short-term symptoms after treatment, but often recurrence. Patients lying on the side, fully exposed perianal skin, with benzidine cleaning skin lesions. With gun liquid nitrogen hypothermia therapy device, press the freezing switch, so that the liquid nitrogen was fine beads uniformly spray in the lesion site, until the formation of a thin layer of skin cream. Treatment of local burning pain, can tolerate, generally do not need anesthesia, such as patients with pain sensitivity can not tolerate, available 0.5% lidocaine or procaine local anesthesia before treatment.
