对80例慢性呼吸衰竭病人和37例健康人的红细胞(RBC)膜带3蛋白结构、含量和阴离子转移功能、RBC内外血气和血中HCO_3~-、Cl~-进行了研究。结果提示:肺心病Ⅱ型呼吸衰竭病人带3蛋白阴离子转运蛋白的氨基酸组分无变化,但带3蛋白阴离子转运功能降低,HCO_3~-/Cl~-交换受限,红细胞内产生的大量HCO_3~-不能充分排出,使红细胞内pH(pHi)升高。RBC内相对偏碱对缓解高碳酸血症所引起的氧离曲线过度右移,具有重要意义。
The structure and content of RBC membrane protein 3 and the function of anion transport in 80 patients with chronic respiratory failure and 37 healthy people were studied. The blood gas inside and outside of RBC and the contents of HCO_3 ~ -, Cl ~ - in RBC were studied. The results suggest that there is no change in the amino acid composition of anion transporter protein 3 in type Ⅱ respiratory failure patients with pulmonary heart disease, but the transport function of anion transporters with 3 protein is reduced, the exchange of HCO 3 ~ - / Cl ~ - is limited, and a large amount of HCO 3 ~ - can not be fully discharged, the intracellular pH (pHi) increased. Relatively alkaline within RBC to relieve hypercapnia caused by excessive shift of the oxygen ion curve is of great significance.
