目的研究振幅整合脑电图(aEEG)联合头颅核磁共振成像(MRI)对痉挛型脑瘫患儿病情及远期预后的评估价值。方法选取2014年1月至2016年2月在该院接受治疗的痉挛型脑瘫患儿69例作为观察对象,分别对所有患儿进行aEEG及头颅MRI检查,同时给予所有患儿相应治疗,并随访1年。分析临床病情程度、MRI分度及远期预后与aEEG改变的相关性。结果随着患儿病情程度不断加重,aEEG检查异常率呈逐渐上升趋势,轻、中、重度异常率比较差异均有统计学意义(P<0.05)。随着MRI分度不断增加,aEEG检查异常率呈逐渐上升趋势,轻、中、重度异常率比较差异均有统计学意义(P<0.05)。aEEG检查正常患儿的正常率为100.00%(25/25),明显高于aEEG检查轻度异常患儿的正常率[79.17%(19/24)],以及aEEG检查重度异常患儿的正常率[0.00%(0/20)],而aEEG检查轻度异常患儿的正常率又明显高于aEEG检查重度异常患儿的正常率,差异均有统计学意义(P<0.05)。经Spearman相关性分析可知,aEEG改变与患儿临床病情程度、MRI分度呈明显正相关关系,与患儿远期预后呈明显负相关关系。结论 aEEG联合头颅MRI检查对痉挛型脑瘫患儿病情及远期预后具有较好的评估价值,值得临床推广应用。
Objective To evaluate the value of aEEG combined with cranial magnetic resonance imaging (MRI) in evaluating the prognosis and prognosis of children with spastic cerebral palsy. Methods Sixty-nine children with spastic cerebral palsy treated in our hospital from January 2014 to February 2016 were enrolled in this study. All children underwent aEEG and cranial MRI examinations, and all children were treated accordingly and followed up 1 year. To analyze the correlation between the degree of clinical condition, MRI index and long-term prognosis and aEEG changes. Results With the increasing severity of illness in children, the abnormal rate of aEEG examination showed a gradual upward trend, and the difference of mild, moderate and severe abnormalities was statistically significant (P <0.05). With the increasing degree of MRI, the abnormal rate of aEEG examination showed a gradual upward trend, and the difference of mild, moderate and severe abnormalities was statistically significant (P <0.05). The normal rate of aEEG in normal children was 100.00% (25/25), which was significantly higher than the normal rate of aEEG in children with mild abnormalities [79.17% (19/24)], and the normal rate of aEEG in children with severe abnormalities [0.00% (0/20)]. However, the normal rate of children with mild abnormalities detected by aEEG was significantly higher than that of children with severe abnormalities detected by aEEG (P <0.05). Spearman correlation analysis shows that the change of aEEG has a positive correlation with the degree of clinical condition and MRI index in children, and has a negative correlation with the long-term prognosis of children. Conclusion aEEG combined with craniocerebral MRI examination of spastic cerebral palsy patients with disease and long-term prognosis has a good evaluation value, it is worthy of clinical application.
