目的观察右美托嘧啶对机械通气患者镇静的效果。方法选择我院2010年10月至2011年5月进行机械通气的成人患者76例,右美托嘧啶在初始10min静脉滴入剂量为1μg/kg,随后的维持剂量为0.2～0.7μg/(kg/h),维持的泵入速率根据所需要的镇静水平、心率、血压等变化随时调整,以获得Ⅱ～Ⅳ级Ramsay镇静评分,同时监测患者用药前及用药后1h、4h持续镇静时呼吸、心率、血压、血氧饱和度的变化。结果 76例患者在负荷剂量下均能够达到镇静效果,Ramsay评分均在Ⅱ～Ⅳ级;在维持剂量时也能保持等效的镇静效果,监测呼吸、循环各指标稳定。结论右美托嘧啶是机械通气患者镇静治疗的有效药物。
Objective To observe the effect of dexmedetomidine on sedation in patients with mechanical ventilation. Methods Seventy-six adult patients with mechanical ventilation from October 2010 to May 2011 in our hospital were enrolled in this study. Dexmedetomidine was intravenously administered at a dose of 1 μg / kg for the first 10 minutes, followed by a maintenance dose of 0.2-0.7 μg / kg / h) to maintain the pump rate according to the required level of sedation, heart rate, blood pressure and other changes at any time to obtain Ⅱ ~ Ⅳ grade Ramsay sedation score, while monitoring patients before and after medication 1h, 4h continuous sedation breathing, Heart rate, blood pressure, oxygen saturation changes. Results All 76 patients achieved sedative effect at loading dose. The Ramsay scores were all in grade Ⅱ ~ Ⅳ. Equivalent sedation was maintained at the maintenance dose. Respiratory and circulatory indexes were monitored for stability. Conclusion Dexmedetomidine is an effective drug for sedation in patients with mechanical ventilation.
