目的:探讨抗 PPD-IgG 测定对结核性脑膜炎的诊断价值。方法:用 ELISA 法对20例结核性脑膜炎和28例非结核性脑膜炎患儿进行脑脊液抗 PPD-IgG 测定并与涂片法找结核杆菌及多聚酶链反应的结果相比较。结果:三种方法诊断结核性脑膜炎的阳性率分别为55%、5%和85%,抗 PPD-IgG 的敏感性高于涂片法(P<0.001)的敏感性,但低于多聚酶链反应的敏感性(P<0.05),并且抗 PPD-IgG 的阳性率随病程延长而增加。结论:脑脊液抗 PPD-IgG 的动态测定可提高结脑诊断的阳性率。
Objective: To investigate the diagnostic value of anti-PPD-IgG for tuberculous meningitis. Methods: Twenty cases of tuberculous meningitis and 28 cases of non-tuberculous meningitis were tested for anti-PPD-IgG in cerebrospinal fluid by ELISA. The results were compared with the results of smear-test for Mycobacterium tuberculosis and polymerase chain reaction. Results: The positive rates of the three methods in diagnosing tuberculous meningitis were 55%, 5% and 85% respectively. The sensitivity of anti-PPD-IgG was higher than that of smear method (P <0.001) Response (P <0.05), and the positive rate of anti-PPD-IgG increased with the duration of the disease. Conclusion: The dynamic test of cerebrospinal fluid anti-PPD-IgG can improve the positive rate of diagnosis of tuberculosis.
