目的探讨心电向量图对阵发性室上性心动过速的鉴别诊断及旁路定位诊断的准确性。方法选择有室上性心动过速病史的患者200例,经食道心房调搏术诱发、终止室上速,并记录心动过速的心电图和心电向量图。在射频消融术中检验心电向量图对隐匿性旁路定位诊断的准确性。结果经心电图检查诊断为室上速的患者有174例,经心电向量图检查诊断为室上速的有196例,二者比较,P<0.05;心电向量图关于左旁路位于左房室间的诊断与射频消融术中的诊断的符合率为100%,右旁路位于右房室间的诊断的符合率为86.71%。结论心电向量图能对室上速做出比较准确的诊断并能对隐匿性旁路作出较精确的定位诊断,在临床上值得推广。
Objective To investigate the diagnostic accuracy of ECG vector chart for the diagnosis of paroxysmal supraventricular tachycardia and the diagnostic accuracy of bypass localization. Methods A total of 200 patients with a history of supraventricular tachycardia were selected and induced by transesophageal atrial pacing. The supraventricular tachycardia was terminated and the tachycardia electrocardiogram and electrocardiogram were recorded. Accuracy of diagnosis of occult bypass in cardiac radiofrequency catheter ablation. Results There were 174 patients diagnosed as supraventricular tachycardia by electrocardiogram and 196 patients were diagnosed as supraventricular tachycardia by electrocardiogram. There was a significant difference between the two groups (P <0.05) The diagnostic accuracy of interventricular and radiofrequency ablation was 100%, and that of right bypass in the right atrioventricular compartment was 86.71%. Conclusion ECG vector map can make a more accurate diagnosis of supraventricular tachycardia and can make a more accurate positioning diagnosis of occult bypass, which is worthy of clinical promotion.
