目的 了解健康活体人眼在自发视远视近调节中 ,睫状体形态和位置动态变化的特点。方法 利用超声生物显微镜 (UBM)的 5 0MHz高频换能器 ,对 6 8例正常人眼注视远、近不同视标时睫状体形态、位置的变化进行动态观测和统计学分析。结果 (1)视近调节时 ,睫状体前部厚度、睫状突长度、睫状体总面积和环形肌面积增大 ,睫状突宽度及内尖成角、睫状突与小梁、虹膜、巩膜突的距离 ,以及其和虹膜、巩膜的夹角减小 ,前房角开放程度变小。而睫状体后部厚度、纵行肌面积、巩膜虹膜夹角以及小梁虹膜夹角的差异无显著意义。 (2 )形态各参数间的变化相辅相成、高度一致 ,位置各参数亦然 ,但形态与位置之间关系无显著意义。结论 人眼视近调节时 ,睫状体各部肌纤维协调收缩 ,尤其是环形肌收缩 ,使睫状突变得尖而细长 ,睫状体向前向内移动 ,松弛悬韧带 ,支持Helmholtz的调节松弛学说。UBM可实时观测人眼调节时睫状体各部分形态和位置等的变化情况。
Objective To understand the dynamic changes of ciliary body morphology and position in healthy living human eye during spontaneous hyperopia. Methods Ultrasound biological microscope (UBM) 50MHz high frequency transducer was used to observe and statistically analyze the change of ciliary body shape and position in 68 normal human eyes when the eyes were far and different. Results (1) As the proximal adjustment, the anterior ciliary body thickness, ciliary process length, the total area of the ciliary body and the ring muscle area increased, ciliary process width and the tip of the angle, ciliary processes and trabecular, Iris, scleral protrusion distance, and its iris, sclera angle decreases, the degree of opening of the anterior chamber angle becomes smaller. There was no significant difference between the posterior ciliary body thickness, the longitudinal muscle area, the angle of sclera iris and the angle of trabecular iris. (2) The morphological changes of all the parameters are mutually complementary and highly consistent, and the parameters of the position are also the same, but the relationship between the shape and the position has no significant meaning. CONCLUSIONS: In the regulation of human visual acuity, the coordination and contraction of muscle fibers of ciliary body, especially the contraction of the ring muscle, make the ciliary processes sharp and slender, the ciliary body moves forward and inward, relax the suspensory ligaments and support the relaxation of Helmholtz theory.
