目的探讨胸腔内注入A型链球菌制剂和顺铂治疗恶性胸水的方法和疗效。方法经病理细胞学证实的恶性胸腔积液49例,经采用中心静脉管行胸腔引流术排尽胸腔积液后,随机分两组,治疗组:26例患者采用胸腔内注入顺铂及A型链球菌制剂,对照组:23例患者仅用顺铂胸腔内注入,2周后复查,最多3次观察疗效及毒副反应。结果总有效率:治疗组为84.6%,明显高于对照组的52.2%(P<0.05),治疗组仅发热较对照组多,但差异无统计学意义(P>0.05)。而胃肠道反应,骨髓抑制差异有统计学意义(P<0.05)。结论A型链球菌制剂与顺铂联合腔内注射治疗恶性胸水是有效低毒可行的方法,较单药顺铂疗效好。
Objective To investigate the methods and efficacy of intrapleural injection of streptococcus pneumoniae and cisplatin in the treatment of malignant pleural effusion. Methods Forty-nine cases of malignant pleural effusion confirmed by pathology cytology were divided into two groups at random after thoracic drainage by central venous catheter. The treatment group: 26 patients received intrapleural injection of cisplatin and type A Streptococcus preparation and control group: 23 patients were injected intrapleurally with Cisplatin alone, 2 weeks later, up to 3 times to observe the curative effect and toxicity. Results The total effective rate was 84.6% in the treatment group, which was significantly higher than that in the control group (52.2%, P <0.05). The treatment group only had more fever than the control group, but the difference was not statistically significant (P> 0.05). The gastrointestinal reaction, bone marrow suppression difference was statistically significant (P <0.05). Conclusion Combination of streptococcus pneumoniae and cisplatin in the treatment of malignant pleural effusion is an effective and low toxicity method, which is more effective than cisplatin alone.
