[目的]观察谷氨酰胺和精氨酸的免疫治疗对肺癌放疗患者T淋巴细胞亚群的影响。[方法]收集临床确诊的非小细胞肺癌病例36例,随机分为单独放疗组(18例)和联合治疗组(18例)。单独放疗组给予单独放疗,联合治疗组放疗同时第1~10d静脉滴注精氨酸和谷氨酰胺。比较两组不同放疗时段白蛋白、前白蛋白、淋巴细胞数及T淋巴细胞亚群CD3+、CD4+、CD8+和CD4+/CD8+的变化。[结果]单独放疗组CD3+、CD4+细胞百分比随放疗剂量增加而下降,CD8+细胞比例上升,而联合治疗组放疗第15d的CD3+、CD4+细胞比放疗前有升高,CD8+细胞较放疗前下降,CD4+/CD8+升高。与单独放疗组相比,联合治疗组放疗中和放疗后CD4+/CD8+均增高,两组比较差异有统计学意义(P<0.01)。[结论]肺癌放疗中同步应用谷氨酰胺和精氨酸的免疫治疗可以升高CD4+/CD8+,从而提高放疗患者的细胞免疫功能。
[Objective] To observe the effect of immunotherapy with glutamine and arginine on T lymphocyte subsets in patients with lung cancer undergoing radiotherapy. [Methods] Thirty-six cases of clinically diagnosed non-small cell lung cancer were collected and randomly divided into radiotherapy alone group (18 cases) and combination therapy group (18 cases). Radiotherapy alone group given radiotherapy alone, combined with radiotherapy group 1 ~ 10d intravenous infusion of arginine and glutamine. The changes of albumin, prealbumin, lymphocyte count and T lymphocyte subsets CD3 +, CD4 +, CD8 + and CD4 + / CD8 + were compared between the two groups during different radiotherapy periods. [Results] The percentage of CD3 + and CD4 + cells in radiotherapy group decreased with the increase of radiotherapy dose, and the proportion of CD8 + cells increased. In the combination therapy group, CD3 + and CD4 + cells increased 15 days after radiotherapy, CD8 + / CD8 + increase. Compared with the radiotherapy alone group, the combination therapy group radiotherapy and radiotherapy CD4 + / CD8 + were increased, the difference between the two groups was statistically significant (P <0.01). [Conclusions] The concurrent immunotherapy with glutamine and arginine during radiotherapy of lung cancer can increase CD4 + / CD8 +, thereby enhancing the cellular immune function in radiotherapy patients.
