目的:比较ITIФ6.5 mm平台种植体配对常规颈基台与配对宽颈基台用于单牙缺失修复后的边缘骨吸收量。方法:选择50例单牙缺失患者,缺失牙槽嵴解剖条件满足植入宽颈Ф6.5(种植体平台)ITI种植体手术适应症,均行非潜入式种植手术,术后3~6月制取印模,随机采用宽颈或常规颈基台各25例,常规完成金钯合金烤瓷冠修复,并随访18个月,通过X线检查骨的吸收情况。结果:全冠修复后,两组植体及修复体均未发生严重并发症,宽颈基台组在修复后3、6、12、18个月的边缘骨吸收量分别为(0.29±0.08)mm、(0.36±0.07)mm、(0.43±0.10)mm、(0.61±0.09)mm,常规颈基台组在修复后3、6、12、18个月的边缘骨吸收量分别为(0.28±0.11)mm、(0.34±0.12)mm、(0.42±0.14)mm、(0.57±0.16)mm。两组骨吸收量经t检验,差异无显著性(P>0.05),均数排列为:宽颈组(0.42 mm)>常规颈组(0.40 mm)。结论:在具有垂直平台转换设计的ITIФ6.5宽颈种植体上应用常规颈基台,形成主动水平平台转换后,种植体颈周骨吸收量略有减少,但差异无统计学意义。
OBJECTIVE: To compare the marginal bone resorption of ITIФ 6.5 mm platform implants matched with conventional cervical abutments and matched wide-necked abutments for single-tooth defect repair. Methods: Fifty patients with unilateral tooth loss were selected. The anatomic conditions of missing alveolar ridge were selected to meet the indications for surgical implantation of wide-necked Ф6.5 (implant platform) ITI implants. Non-submerged implant surgery was performed. From March to June Impressions were made. 25 cases of wide neck or common cervical abutments were randomly used. The gold-palladium alloy porcelain crowns were routinely repaired and followed up for 18 months. The bone absorption was examined by X-ray. Results: No significant complications occurred in the two groups of implants and prostheses after crown restoration. The marginal bone resorption rates at 3, 6, 12 and 18 months in the wide neck abutment group were (0.29 ± 0.08) mm, (0.36 ± 0.07) mm, (0.43 ± 0.10) mm and (0.61 ± 0.09) mm respectively. The marginal bone resorption at 3, 6, 12 and 18 months in the conventional cervical abutment group was (0.28 ± 0.11 mm, 0.34 ± 0.12 mm, 0.42 ± 0.14 mm, and 0.57 ± 0.16 mm, respectively. There was no significant difference in bone resorption between the two groups (P> 0.05). The mean values were as follows: wide neck group (0.42 mm)> conventional neck group (0.40 mm). CONCLUSION: Conventional cervical abutments were applied to ITIФ6.5 wide-necked implants with vertical platform conversion. After active horizontal platform conversion, the amount of cervical bone resorption decreased slightly, but the difference was not statistically significant.
