自身外周血干细胞移植(APSCT)是重建骨髓功能的一种方法,作者报告了56例伴骨髓异常的复发性霍奇金病(HD)经大剂量环磷酰胺(C)卡氮芥(B),表鬼臼毒甲醛糖甙(V)化疗后行APSCT。其中男38例,女18例,中数年龄32岁(17~51岁),53例经病理证实复发,3例部分缓解后转为进展期,给MOPP及ABVD 化疗无效。56例均有骨髓异常,其中26例骨髓肿瘤细胞浸润,11例有早期浸润表现,17例经盆腔照射(其中3例骨髓有肿瘤细胞浸润),2例骨髓增生低下。于骨髓增生稳定期单采周围血干细胞,重复单采每周不多于五次,直至单个核细胞总量至少达6.5×10~9/kg 止,液氮冷冻保存,加DMSO 防冻。病人入移植房,静脉用C1.5g/m~2/天×d天(-5~-2)。B300mg/m~2第一天(-5),V125~150mg/m~2,每天2次×3天(-5~-3),第0天回输冷冻干细胞。结果3例死于早期毒性反应,CR29例,PR18
Autologous peripheral blood stem cell transplantation (APSCT) is a method of reconstructing bone marrow function. The authors reported that 56 patients with recurrent Hodgkin’s disease (BM) with myelodysplastic syndrome were treated with high-dose cyclophosphamide (C) , Epiglottotin formaldehyde glycosides (V) chemotherapy after APSCT. There were 38 males and 18 females, the median age was 32 years old (17 ~ 51 years old). 53 cases were confirmed by histopathology and 3 cases were partially remitted to advanced stage. MOPP and ABVD chemotherapy were ineffective. All of the 56 cases had myelodysplasia, of which 26 cases had infiltration of bone marrow tumor cells, 11 cases had early infiltration, 17 cases had pelvic irradiation (3 of which had tumor cell infiltration) and 2 cases of bone marrow hyperplasia. Peripheral blood mononuclear hematopoietic stem cells in the stationary phase of myeloproliferation should be repeated apheresis no more than five times per week until the total amount of mononuclear cells reaches at least 6.5 × 10 ~ 9 / kg, liquid nitrogen is cryopreserved and DMSO antifreeze is added.
