目的:探讨降钙素原(PCT)、白细胞介素-6(IL-6)对脓毒症早期的临床诊断价值。方法:根据关键词检索文献后,按照纳入标准和排除标准进行文献筛选,利用R2.15.1软件对收集到的文献数据进行Meta分析。采用异质性检验方法,计算各生物标志物检验脓毒症的敏感度和特异度。结果:根据搜索策略检索到480篇相关文献,筛选后有22篇文献入选,其中包括中文文献13篇,英文文献9篇,异质性检验结果显示各组文献间差异有统计学意义(χ2=33.25,P<0.05),Meta分析结果表明PCT和IL-6是最具有检测价值的生物标记物,其中PCT对脓毒症诊断的敏感度和特异度分别为84.1%和80.2%,而IL-6的相应数据分别为80.2%和77.4%。结论:PCT对脓毒症的早期诊断具有较高的临床价值,同时配合IL-6诊断,能够进一步提高对脓毒症早期诊断的敏感度和特异度。
Objective: To investigate the clinical value of procalcitonin (PCT) and interleukin-6 (IL-6) in the early diagnosis of sepsis. Methods: After searching the documents according to keywords, the documents were screened according to inclusion criteria and exclusion criteria. Meta-analysis was performed on the collected literatures using R2.15.1 software. Heterogeneity tests were used to calculate the sensitivity and specificity of each biomarker for sepsis testing. Results: According to the search strategy, 480 related literatures were retrieved. Twenty-two literatures were selected after screening, including 13 Chinese literatures and 9 English literatures. The results of heterogeneity test showed that there was significant difference between the groups (χ2 = 33.25, P <0.05). The results of Meta analysis showed that PCT and IL-6 were the most biomarkers of detection value. The sensitivity and specificity of PCT for sepsis diagnosis were 84.1% and 80.2% The corresponding data for 6 are 80.2% and 77.4%, respectively. Conclusion: PCT has high clinical value in the early diagnosis of sepsis, and combined with IL-6 diagnosis can further improve the sensitivity and specificity of early diagnosis of sepsis.
