目的:观察丹蒌片对动脉粥样硬化(AS)患者血脂,外周血C-反应蛋白(CRP),核转录因子-κB(NF-κB)水平及颈动脉病变的影响,探讨其抗动脉粥样硬化作用及相关机制。方法:符合入选标准的87例患者随机分为对照组和丹蒌片组(治疗组),对照组给予瑞舒伐他汀治疗,治疗组在对照组治疗的基础上加用丹蒌片。12周后全自动生化仪检测血清总胆固醇(TC),总甘油三酯(TG),高密度脂蛋白(HDL),低密度脂蛋白(LDL),脂蛋白a(LPa),载脂蛋白A(Apo A),载脂蛋白B(Apo B);乳胶免疫比浊法,酶联免疫吸附法(ELISA)检测外周血CRP,NF-κB水平;飞利浦ie33超声机检测颈动脉内膜、中膜厚度(inteima-media thickness,IMT)并进行颈动脉AS斑块积分。结果:与治疗前比较,两组血清TC,TG,LDL-C,LPa,Apo A,Apo B,CRP,NF-κB,CRP,IMT厚度及斑块积分水平均降低,血清Apo A,HDL水平升高(P<0.05,P<0.01),但对照组HDL升高无统计学意义。治疗后与对照组比较,治疗组血清TC,LDL-C,LPa,Apo B降低更显著(P<0.05);治疗组hs-CRP,NF-κB表达减少更明显(P<0.01);治疗组颈动脉IMT厚度及斑块积分下降优于对照组(P<0.05)。结论:丹蒌片联合瑞舒伐他汀能有效调节血脂水平,干预颈动脉AS斑块的发生发展,其抗AS的作用机制可能与下调CRP,NF-κB等炎症因子水平有关。
OBJECTIVE: To observe the effect of dancao tablet on blood lipids, C-reactive protein (CRP), nuclear factor-κB (NF-κB) levels and carotid artery disease in patients with atherosclerosis (AS) Like sclerosis and related mechanisms. Methods: Eighty-seven patients who met the inclusion criteria were randomly divided into control group and Danban Tablet group (treatment group). The control group was treated with rosuvastatin. The treatment group was treated with dander tablet on the basis of the control group. After 12 weeks, the levels of total cholesterol (TC), total triglyceride (TG), high density lipoprotein (HDL), low density lipoprotein (LDL), lipoprotein (LPa), apolipoprotein A A), Apo B, latex immunoturbidimetric assay and enzyme-linked immunosorbent assay (ELISA) were used to detect the level of CRP and NF-κB in peripheral blood. The carotid intima and media thickness Integra-media thickness (IMT) and carotid artery AS plaque score. Results: The levels of TC, TG, LDL-C, LPa, Apo A, Apo B, CRP, NF-κB, CRP and IMT in the two groups were significantly lower than those before treatment (P <0.05, P <0.01), but the control group had no significant increase of HDL. Compared with the control group, the levels of TC, LDL-C, LPa and Apo B in the treatment group were significantly decreased (P <0.05), and the expressions of hs-CRP and NF-κB in the treatment group decreased more obviously Carotid IMT thickness and plaque score decreased better than the control group (P <0.05). Conclusion Dan Dan tablets combined with rosuvastatin can effectively regulate blood lipid levels and interfere with the development of carotid artery AS plaque. The anti-AS mechanism may be related to the downregulation of inflammatory factors such as CRP and NF-κB.
