为探讨胃癌组织幽门螺杆菌(Hp) 感染和nm23 表达及胃癌根治术后再发癌之间的关系,用Warrthin- Starry方法检测胃组织Hp 感染,采用免疫组化S- P法检测64 例胃癌组织nm23 表达,并结合内镜及随访资料进行分析。胃癌Hp 感染率、nm23 低表达率在淋巴结转移组及术后3 年内有再发癌组明显增高。而且nm23 低表达与肿瘤浸润程度有关,浸及浆膜及周围脏器组nm23 低表达率为78-6% ,浸及粘膜及粘膜下者为40-0 % ( P <0-05) 。Hp 感染者nm23 低表达率明显高于Hp 阴性组。Hp 在胃癌发生、发展过程中起着重要作用。它可能通过影响癌基因蛋白的表达起促癌作用。胃癌nm23 低表达者具有较强的浸润转移能力,且术后易发生再发癌,nm23 表达变化对判断胃癌术后再发癌的发生及预后有重要意义。
To investigate the relationship between Helicobacter pylori (Hp) infection and nm23 expression in gastric cancer tissues and the recurrence of cancer after radical gastrectomy, the gastric tissue Hp infection was detected by the Warrthin-Starry method. 64 cases of gastric cancer were detected by immunohistochemical S-P method. Tissue nm23 expression was combined with endoscopy and follow-up data for analysis. The rate of Hp infection and low expression of nm23 in gastric cancer were significantly higher in the lymph node metastasis group and in the recurrent cancer group within 3 years after surgery. Furthermore, the low expression of nm23 was associated with the degree of tumor infiltration. The low expression rate of nm23 in the plasma membrane and surrounding organs was 78-6%, and it was 40-0 % in the mucosa and submucosa (P<0-05). The low expression rate of nm23 in Hp infected patients was significantly higher than that in Hp negative group. Hp plays an important role in the occurrence and development of gastric cancer. It may promote cancer by affecting the expression of oncogene proteins. Those with low expression of nm23 in gastric cancer have strong ability of invasion and metastasis, and are prone to recurrent cancer after surgery. The expression of nm23 is important for judging the occurrence and prognosis of recurrence of gastric cancer.
