腹腔镜结扎精索内静脉曲张是一种比较新颖的操作.早期效果显示这是对有症状的患者一种切实可厅的治疗手段.本文报道107例在腹股管内环近侧结扎曲张静脉的方法.手术指征为:低生育力98例,睾丸疼痛9例.诊断经由多普勒超声检查证实.72%患者为左侧曲张,27%为双侧病变.采用三点穿刺法及CO_2注入腹腔.精索静脉——游离后,以钛夹夹住,精索动脉全部保留.手术后患者当日回家.手术时问平均61.4分钟(单侧手术56.6分,双侧75.8分),并发症少,两例发生阴囊气肿,另两例切口感染,61例在手术前后作精液检查显示术后精子数和活动力改善,同时形态不正常精子百分率下降.
