目的了解宁波市妊娠早、中、晚期孕妇甲状腺激素水平。方法选取2013年10月—2014年11月在宁波市海曙区、江东区、江北区、鄞州区、慈溪市和宁海县妇幼保健门诊进行初次产检建册的健康单胎妊娠早期孕妇5 616人、妊娠中期孕妇339人、妊娠晚期孕妇187人和182名非妊娠正常育龄妇女(对照组),采用化学发光免疫测定法检测甲状腺素(FT4)、促甲状腺素(TSH)、甲状腺球蛋白抗体(Tg Ab)和甲状腺过氧化物酶抗体(TPOAb)4项指标水平。结果四组TSH、FT4和Tg Ab水平比较,差异均有统计学意义(P<0.01);四组TPOAb水平比较,差异无统计学意义(P>0.05)。两两比较结果显示,TSH水平由高到低依次为对照组、妊娠晚期组、妊娠中期组和妊娠早期组(均P<0.05);妊娠中期及妊娠晚期组FT4水平低于对照组(均P<0.01);妊娠早期、中期和晚期组患者Tg Ab水平均高于对照组(P<0.01)。结论宁波地区不同妊娠期孕妇的TSH、FT4和Tg Ab水平有所不同,除Tg Ab外,各项甲状腺激素水平均在正常参考范围内。
Objective To understand the levels of thyroid hormone in pregnant women in early, middle and late stages of pregnancy in Ningbo. Methods From October 2013 to November 2014, a total of 5 616 healthy pregnant women with singleton pregnancies were enrolled in the MCH clinics in Haishu District, Jiangdong District, Jiangbei District, Yinzhou District, Cixi City and Ninghai County in Ningbo City. 339 pregnant women in the second trimester of pregnancy, 187 pregnant women in the third trimester of pregnancy and 182 normal pregnant women of childbearing age (control group). The levels of thyroid hormone (FT4), thyrotropin (TSH), thyroglobulin antibody Ab) and thyroid peroxidase antibody (TPOAb) 4 indicators level. Results The levels of TSH, FT4 and Tg Ab in the four groups were significantly different (P <0.01). There was no significant difference in TPOAb levels between the four groups (P> 0.05). The results of comparison showed that the level of TSH was highest in the control group, the third trimester group, the second trimester group and the third trimester group (all P <0.05); the levels of FT4 in the third trimester and the second trimester were lower than those in the control group <0.01). The levels of Tg Ab in the early, middle and late pregnancy groups were significantly higher than those in the control group (P <0.01). Conclusion The levels of TSH, FT4 and Tg Ab in pregnant women in different gestation stages in Ningbo are different. The levels of thyroid hormones except Tg Ab are within the normal reference range.
