目的调查北京市朝阳区男男性行为者(men who have sex with men,MSM)的人类免疫缺陷病毒(HIV)的感染情况,并分析其影响因素,为今后开展宣教干预工作提供依据。方法通过网络宣传、性伴动员等方式招募MSM,对其进行问卷调查及HIV抗体检测。采用SPSS 21.0进行统计分析。结果共招募MSM 259人,确证HIV感染22人,阳性率为8.5%。259名MSM的平均年龄为29.7岁;未婚者占83.4%,大专以上学历占80.3%,非北京户籍占68.7%。首次发生男性插入式性交的平均年龄是22.7岁。83.8%的被调查者近1个月内发生过插入式性行为,其中坚持全程使用安全套者占59.9%;近3个月内,与同性商业性工作者发生性交者占4.5%。19.7%的被调查者从未做过HIV检测,20.1%的被调查者近1年内吸烟;5.0%的被调查者自述在近1年内被诊断为性病。多因素分析结果显示,最近1年内吸烟、最近1个月发生过1~3次无保护性行为、HIV检测不定期或从未做过HIV检测以及近1年内被诊断为性病会增加感染HIV的风险。结论需加强对朝阳区MSM的宣教和干预,减少其无保护性行为发生率,提高其HIV抗体规范检测率,预防艾滋病在该人群的传播。
Objective To investigate the prevalence of human immunodeficiency virus (HIV) infection in men who have sex with men (MSM) in Chaoyang District, Beijing, and to analyze the influencing factors, so as to provide basis for carrying out mission intervention in the future. Methods The MSM was recruited through online publicity and sexual mobilization, and questionnaires and HIV antibody testing were conducted. SPSS 21.0 statistical analysis. Results A total of 259 MSM were recruited to confirm HIV infection in 22 people, the positive rate was 8.5%. The average age of 259 MSM was 29.7 years old; unmarried persons accounted for 83.4%, college education 80.3%, non-Beijing household registration 68.7%. The average age for first-time male sexual intercourse is 22.7 years. 83.8% of the respondents have had sexual intercourse within the past month, among which 59.9% insist on using condom in the whole process and 4.5% have sex with same-sex commercial workers in the past 3 months. 19.7% of the respondents have never done HIV testing, 20.1% of respondents smoked within the past year; 5.0% of respondents read the report in the nearly 1 year was diagnosed as sexually transmitted diseases. Multivariate analysis showed that smoking in the recent one year had had 1 or 3 unprotected sex in the last month, and HIV testing was not done regularly or had never been tested before and HIV was diagnosed as STD in the past year risk. Conclusion It is necessary to strengthen the mission and intervention of MSM in Chaoyang District, reduce the incidence of unprotected sex, increase the detection rate of HIV antibody and prevent the spread of AIDS in this population.
