①目的观察自行设计的活性同种血管(VHA)在肺外科手术应用的效果。②方法全组4例,包括左肺上叶肺癌侵及左肺动脉,行VHA-病变肺动脉替换3例;右肺中下叶肺癌切除术中分离胸膜肺粘连误伤右无名静脉汇入上腔静脉处,以VHA移植重建1例。③结果1例因移植血管扭曲、梗阻,术后26d再次手术切除余肺;另3例术后恢复顺利。无手术死亡。随访1.0~2.5年,1例术后1.0年死于肺癌复发转移,1例术后2.5年死于脑溢血,第3例至今存活已1.3年,未见复发。术后胸部X线检查均显示余肺膨胀良好。④结论在肺血管缺失情况下VHA是一种良好替代材料,其应用扩大了肺癌切除适应证,改善了外科治疗效果
①Objective To observe the effect of self-designed active allogenic blood vessels (VHA) in pulmonary surgery. Methods The whole group consisted of 4 patients, including the left upper lobe lung cancer invading the left pulmonary artery and the VHA-lesion pulmonary artery replacement in 3 cases. The right lower lobe lobectomy lung cancer resection removing the pleural adhesions and injuring the right anonymous vein into the superior vena cava One case was reconstructed with VHA. ③ Results One case was distorted and obstructed by transplanted blood vessel, and the remaining lung was resected 26 days after operation. The other three cases recovered smoothly. No surgery died. Follow-up 1.0 ~ 2.5 years, 1 patient died of lung cancer recurrence and metastasis after 1 year, 1 patient died of cerebral hemorrhage 2.5 years after surgery, the third case has survived for 1.3 years, no recurrence . Chest X-ray showed good lung expansion. Conclusion VHA is a good alternative material in the absence of pulmonary vasculature, and its application expands the indications for lung cancer resection and improves the surgical treatment effect
