目的:探讨倦怠治疗对中青年男性2型糖尿病患者糖代谢控制及慢性并发症的影响。方法:采用国际通用的M aslach倦怠量表(MB I-GS)对284例患者进行倦怠治疗程度的调查,观察倦怠治疗对糖代谢控制及慢性并发症的影响。结果:284例中青年2型糖尿病患者倦怠治疗处于偏高水平,35.2%的患者存在一定程度的情绪耗竭,治疗依从性差,其中重度占12.7%,轻度占22.5%;23.6%的患者存在去人格化,其中重度占9.2%,轻度占14.4%;24.6%的患者缺乏工作成就感,其中重度占8.1%,轻度占16.5%;情绪衰竭和低成就感对糖代谢影响较显著,与正常组相比,轻、重度情感衰竭和低成就感患者血糖水平及GHbA1 c水平显著高于正常患者(P<0.05,P<0.01);而去人格化程度对糖代谢影响较小,仅在重度去人格化患者中血糖水平及GHbA1 c水平中有所升高(P<0.05);情绪衰竭和低成就感对糖尿病大血管和微血管并发症影响较显著(P<0.05);去人格化程度对糖尿病大血管和微血管并发症影响不显著。结论:中青年2型糖尿病患者倦怠治疗处于偏高水平,护理人员应采取有力措施做好健康教育,提高治疗依从性,预防中青年糖尿病患者倦怠治疗状态。
Objective: To investigate the effect of burnout treatment on glycometabolism and chronic complications in middle-aged and young men with type 2 diabetes mellitus. Methods: A total of 284 patients undergoing malaria burnout scale (MB I-GS) were used to investigate the degree of burnout. The effects of burnout treatment on glycometabolism and chronic complications were observed. Results: 284 middle-aged patients with type 2 diabetes mellitus had a high level of burnout treatment, 35.2% of patients had some degree of emotional exhaustion and poor treatment compliance, of which 12.7% were severe and 22.5% were mild; 23.6% Of them, 9.2% were moderate and 14.4% were mild, while 24.6% of them lacked a sense of job satisfaction, 8.1% of which were severe and 16.5% of them mildly. Emotional failure and low accomplishment had a significant effect on glucose metabolism, (P <0.05, P <0.01). However, the degree of depersonalization had little effect on glucose metabolism, only in patients with mild to severe emotional failure and low accomplishment, the level of GHGA1c was significantly higher than that in normal patients (P <0.05). The emotional exhaustion and low accomplishment had significant effects on the macrovascular and microvascular complications in diabetic patients (P <0.05). The degree of depersonalization had no effect on diabetic macrovascular And microvascular complications have no significant effect. Conclusion: The treatment of burnout in middle-aged and young patients with type 2 diabetes is at a high level. Nurses should take effective measures to do well in health education, improve treatment compliance and prevent burnout treatment in middle-aged and young patients with diabetes.
