目的 评价影响 2型糖尿病患者周围神经传导速度的有关因素。方法 对 15 7例 2型糖尿病患者和 40例正常对照者进行周围神经传导速度测定 ,并就其与病程、尿白蛋白排泄和糖化血红蛋白 (HbA1c)的关系进行分析。结果 ①在病程 <5年、5~ 10年、10~ 15年和 >15年的患者中 ,周围感觉神经电生理异常的发生率随病程的延长而显著增高 ,P <0 .0 5或 0 .0 1,而运动神经病变早期多不明显 ,仅在病程 >10年的患者中其神经传导速度才明显减慢 ;②在正常、微量和大量白蛋白尿 ,感觉神经电生理异常的发生率随尿白蛋白排泄的增加而增加 ,大量白蛋白尿患者周围运动神经波幅明显降低 ,传导速度显著减慢 ;③与正常对照组比较 ,HbA1c显著升高 ,P <0 .0 1;在病程相同的组内分析 ,与无神经病变者相比 ,伴神经病变者HbA1c明显升高 ,P <0 .0 5。结论 糖尿病患者周围神经病变与病程、尿白蛋白排泄和血糖控制有关
Objective To evaluate the influencing factors of peripheral nerve conduction velocity in type 2 diabetic patients. Methods Peripheral nerve conduction velocity was measured in 15 7 type 2 diabetic patients and 40 normal controls, and their relationship with duration, urinary albumin excretion and HbA1c were analyzed. Results ①The incidence of peripheral neurosensory abnormalities increased with the duration of the disease in patients with disease duration <5 years, 5 to 10 years, 10 to 15 years and> 15 years, P <0.05 or 0 .0 1, and motor neuropathy was not obvious early, only in patients with disease duration> 10 years, the nerve conduction velocity was significantly slowed down; ② in normal, trace and large amounts of albuminuria, the incidence of sensory nerve electrophysiology abnormalities With the increase of urinary albumin excretion, a large number of patients with albuminuria around the motor nerve amplitude was significantly reduced, conduction velocity significantly slowed; ③ Compared with the normal control group, HbA1c significantly increased, P <0. 01; the same course of disease In the group analysis, HbA1c was significantly higher in patients with neuropathy than in those without neuropathy (P <0.05). Conclusions Peripheral neuropathy in diabetic patients is related to the course of disease, urinary albumin excretion and glycemic control
