目的:探讨检测支原体肺炎(MPP)患儿血清可溶性细胞间粘附分子-1(sICAM-1)及心肌酶谱在急性期与恢复期的变化和临床意义。方法:检测60例MPP患儿在急性期与恢复期血清sICAM-1及心肌酶谱的水平,同时与60例健康体检儿的相应检测结果进行比较。结果:①MPP患儿急性期血清sICAM-1、心肌酶谱水平明显高于恢复期和正常对照组(P<0.01);②MPP患儿恢复期血清sICAM-1、心肌酶谱水平与对照组相比无统计学差异(P>0.05);③60例MPP患儿急性期出现心肌酶异常(生化指标1项以上升高)48例,占80.00%。其中乳酸脱氢酶增高29例,占48.33%;磷酸肌酸激酶增高21例,占35.00%;门冬氨酸转氨酶增高17例,占28.33%;肌酸激酶同工酶增高12例,占20.00%;④急性期心肌酶异常的48例MPP患儿血清sICAM-1水平均高于心肌酶正常的12例MPP患儿(P<0.01)。结论:MPP患儿急性期大多有不同程度的心肌细胞损害,急性期存在心肌损害的MPP患儿血清sICAM-1水平明显增高,故急性期联合检测血清sICAM-1及心肌酶谱的水平有利于早期发现、早期诊断MPP患儿的心肌损害,及时加用营养心肌的药物,以免引起心肌的永久性损害。
Objective: To investigate the changes and clinical significance of serum soluble intercellular adhesion molecule-1 (sICAM-1) and myocardial enzymes in acute and convalescent patients with mycoplasma pneumonia (MPP). Methods: The levels of serum sICAM-1 and myocardial enzymes in 60 MPP children during acute phase and convalescent phase were detected, and compared with the corresponding results of 60 healthy children. Results: ①The levels of sICAM-1 and myocardial enzymes in acute phase of MPP were significantly higher than those in recovery and normal control groups (P <0.01). ②The levels of sICAM-1 and myocardial enzymes in convalescent phase of MPP were significantly higher than those in control group There was no statistical difference (P> 0.05). ③Among the 60 MPP children, 48 patients (80.00%) had abnormal myocardial enzymes (1 biochemical index increased) in acute phase. Lactate dehydrogenase increased in 29 cases, accounting for 48.33%; creatine phosphokinase increased in 21 cases, accounting for 35.00%; aspartate aminotransferase increased in 17 cases, accounting for 28.33%; creatine kinase isoenzyme increased in 12 cases, accounting for 20.00 %; (4) Serum levels of sICAM-1 in 48 MPP children with abnormal myocardial enzymes were higher than those in 12 MPP patients with normal myocardial enzymes (P <0.01). CONCLUSIONS: Most MPP children have different degrees of cardiomyocyte damage in acute phase. The level of serum sICAM-1 in MPP children with acute myocardial injury is significantly increased. Therefore, the serum levels of sICAM-1 and myocardial enzymes in acute MPP patients are favorable Early detection, early diagnosis of myocardial damage in children with MPP, the timely use of nutrition myocardial drugs, so as not to cause permanent myocardial damage.
