目的:99m Tc M I B I可被具代谢活性的肿瘤细胞被动摄取且可在 P糖蛋白( Pgp)作用下被泵出肿瘤细胞。 Pgp 是一种分子量为 170ku 的膜蛋白,被 M D R1 基因编码,是某些抗肿瘤药物的能量依赖性的流出泵。因此,99m Tc M I B I S P E C T 是否可以预测肿瘤患者的化疗反应受到关注。方法:选择 16 例未经化疗和4 例已接受化疗的脑肿瘤患者行99m Tc M I B I S P E C T 显像,并参照磁共振显像,与201 Tl S P E C T 显像进行比较。考察肿瘤的 M D R1 基因表达及 Pgp 含量, 并与99m Tc M I B I S P E C T 比较。结果:99m Tc M I B I与 201 Tl一样,在恶性胶质瘤区浓聚并潴留。99m Tc M I B I的影像比201 Tl 清晰,对比度好。半定量分析示未经治疗患者早期摄取指数( U I,早期病变处与对侧区域的平均计数/象素的比值)与恶性度有关。低分化胶质瘤为 108±006,未分化星形细胞瘤为 410±084,胶质瘤为 571±347,脑转移瘤为 752±152。脑转移瘤患者的99m Tc M I B I潴留指数
Objective: 99m Tc M I B I can be passively metabolized by tumor cells and can be pumped out of tumor cells under the action of P-glycoprotein (Pgp). Pgp is a membrane protein with a molecular weight of 170 ku, encoded by the M DR-1 gene and an energy-dependent efflux pump for certain anti-tumor drugs. Therefore, whether 99m Tc M I B I S P E C T can predict chemotherapy response in cancer patients has drawn attention. Methods: Totally 99m Tc-M I B I S P E C T imaging was performed in 16 patients without chemotherapy and in 4 patients who had undergone chemotherapy. MRI was compared with magnetic resonance imaging and with 201 Tl S P E C T Like to compare. M D R 1 tumor gene expression and Pgp content, and 99m Tc M I B I S P E C T comparison. RESULTS: 99m Tc-M I B I, like 201 Tl, accumulated and retained in the glioblastoma area. 99m Tc M I B I image than 201 Tl clear, good contrast. Semiquantitative analysis showed that early untreated patients uptake index (UI, early lesion and the contralateral region of the average count / pixel ratio) and malignancy. Poorly differentiated glioma was 1 08 ± 0 06, undifferentiated astrocytoma was 4 10 ± 0 84, glioma was 5 71 ± 3 47, brain metastases was 7 52 ± 1 52. 99m Tc M I B I retention index in patients with brain metastases
