目的初步探讨多层螺旋CT(MSCT)血管成像显示颈部动脉狭窄的价值。方法临床可疑或诊断脑供血不足的24例病人行颈部动脉MSCT检查,其结果与导管法颈部动脉造影对照。结果 24例病人共168段颈部动脉中,导管法造影显示正常或轻度狭窄为147段,MSCT对其中的 139段颈部动脉狭窄程度分级与前言一致;导管法造影显示中度或中度以上狭窄(≥30%)为21段, MSCT对其中的19段颈部动脉狭窄程度分级与前者一致,MSCT显示颈部动脉中度和中度以上狭窄的敏感度、特异度、阳性预测值和阴性预测值分别为90.5%、94.6%、70.1%和98.6%。在168段颈部动脉中,导管法造影显示正常、轻度或中度狭窄为156段,MSCT对其中的1 54段颈部动脉狭窄程度分级与前者一致;导管法造影显示高度狭窄或闭塞(≥70%)为12段,MSCT对12段颈部动脉狭窄程度分级与前者一致,MSCT显示颈部动脉重度狭窄和闭塞的敏感度、特异度、阳性预测值和阴性预测值分别为 100%、98.7%、85.7%和100%。结论 MSCT 血管成像显示颈部动脉中、高度狭窄的准确性很高,可以部分替代导管法颈部动脉造影。
Objective To investigate the value of multi-slice spiral CT (MSCT) angiography in the diagnosis of cervical artery stenosis. Methods Twenty-four patients with clinically suspected or insufficient cerebral blood supply underwent MSCT of the neck arteries. The results were compared with catheterization of the neck artery. Results Of the 168 patients with a total of 168 segments of the arteries in the neck, catheterization showed normal or mild stenosis of 147 segments. Conclusion MSCT angiography showed high accuracy of the stenosis in the neck arteries and could partly replace catheterization of the neck arteries.
