患者,男46岁,工人。主诉:一年前因咽部不适,颈部淋巴结肿大,在外院病检为低分化鳞癌,后取鼻咽粘膜病检诊为鼻咽癌淋巴结转移。经钴60及化疗后病情好转,近来左上肢及右下肢阵发性疼痛,有时难以忍受住院。查体:体温、脉搏正常,血压14.7/9.3kPa。发育正常,营养中等,痛苦病容,神清合作,皮肤巩膜无黄染,颈部淋巴结触及不满意,其余淋巴结未触及。颜面及眼睑浮肿,瞳孔圆形等大,光反应灵敏。胸部无畸形,心肺无特殊。腹软,肝脾未触及。
Patient, male 46 years old, worker. Chief complaint: One year ago, due to discomfort in the throat, lymph node enlargement in the neck, poorly differentiated squamous cell carcinoma in the external hospital, and nasopharyngeal mucosal disease after examination for lymph node metastasis of nasopharyngeal carcinoma. After Cobalt 60 and chemotherapy improved, the paroxysmal pain in the left upper extremity and right lower extremity has recently occurred and it is sometimes difficult to tolerate hospitalization. Physical examination: body temperature, normal pulse, blood pressure 14.7/9.3 kPa. Normal development, moderate nutrition, painful disease, Shen Qing cooperation, no yellow stain on the skin sclera, unsatisfactory cervical lymph node touch, and other lymph nodes not touched. The face and eyelids are edema, and the pupil is round and large. The photoreaction is sensitive. No deformity in the chest, no special heart and lung. The abdomen is soft and the liver and spleen are not touched.
