目的:观察苍附调经汤联合西药治疗多囊卵巢综合征(PCOS)的临床疗效。方法:将95例肾虚痰湿型PCOS患者按随机数字表法分为对照组47和实验组48例。2组均采用炔雌醇环丙孕酮片和枸橼酸氯米芬片治疗,实验组加用苍附调经汤内服,2组疗程均为4个月经周期。观察2组子宫内膜厚度、子宫内膜类型、宫颈黏液评分及排卵、月经恢复、妊娠情况;检测治疗前后黄体生成素(LH)、睾酮(T)、雌二醇(E2)和卵泡刺激素(FSH)水平。结果:治疗后,2组子宫内膜厚度和宫颈黏液评分均较治疗前增加(P<0.01);实验组子宫内膜厚度厚于对照组(P<0.01),宫颈黏液评分高于对照组(P<0.01)。2组A型子宫内膜所占比例均较治疗前上升(P<0.05),实验组A型子宫内膜占72.92%,高于对照组的55.32%(P<0.05)。实验组妊娠率为60.42%,高于对照组的40.43%(P<0.05);2组T、FSH、LH水平均较治疗前下降(P<0.01),实验组T、LH水平均低于对照组(P<0.01);2组E2水平均较治疗前上升(P<0.01),2组间比较,差异无统计学意义(P>0.05)。结论:苍附调经汤联合西药治疗肾虚痰湿型PCOS患者,可增加患者的子宫内膜厚度,改善内膜形态,促进月经恢复,调节内分泌状态,从而提高妊娠率。
Objective: To observe the curative effect of Cansichaojing Decoction combined with western medicine on polycystic ovary syndrome (PCOS). Methods: Ninety-five patients with phlegm-dampness-type PCOS were divided into control group 47 and experimental group according to random number table method. Both groups were treated with ethinyl estradiol cyproterone tablets and clomiphene citrate tablets. The experimental group was treated with Cailian tiaojing decoction orally, and the two courses of treatment were 4 menstrual cycles. The changes of endometrial thickness, endometrial type, cervical mucus score and ovulation, menstruation recovery and pregnancy were observed in two groups before and after treatment. The levels of LH, T, E2 and follicle stimulating hormone (FSH) level. Results: After treatment, the thickness of endometrium and cervical mucus score in both groups were significantly higher than those before treatment (P <0.01); the thickness of endometrium in experimental group was thicker than that in control group (P <0.01); the score of cervical mucus was higher than that in control group P <0.01). The proportion of type A endometrium in both groups was higher than that before treatment (P <0.05). The type A endometrium in experimental group accounted for 72.92%, which was 55.32% higher than that in control group (P <0.05). The pregnancy rate in the experimental group was 60.42%, which was higher than that in the control group (40.43%, P <0.05). The levels of T, FSH and LH in the two groups were lower than those before treatment (P <0.01) (P <0.01). The levels of E2 in both groups were significantly higher than those before treatment (P <0.01). There was no significant difference between the two groups (P> 0.05). Conclusion: Cansichaojing Decoction combined with western medicine can improve the pregnancy rate by increasing the endometrial thickness, improving the endometrial morphology, promoting the recovery of menstruation, regulating the endocrine state, and improving the pregnancy rate.
