目的探讨超声引导下射频消融(radiofrequencyablation,RFA)治疗甲状腺良性结节的临床疗效。方法 2014年6月~2015年10月对35例甲状腺良性结节在局部麻醉下行超声引导下RFA治疗,将18G射频针在彩超引导下置入甲状腺肿瘤的中心部位,启动射频仪器,开始对结节进行消融。输出功率3W,消融时间因肿瘤的大小及成分不同而不同,一般为3~20 min。结果所有病例均成功行RFA,术中无明显并发症发生,术后7 d内1例出现穿刺部位感染。RFA术后6个月34.2%(12/35)患者结节全部吸收,48.5%(17/35)结节体积较术前缩小≥50%,17.1%(6/35)结节体积较术前缩小25%~50%。结论超声引导下RFA治疗甲状腺良性结节短期效果显著,临床疗效好,是一种值得临床推广的手术方式,但是长期效果还有待于继续随访及评价。
Objective To investigate the clinical efficacy of ultrasound-guided radiofrequency ablation (RFA) in the treatment of benign thyroid nodules. Methods From June 2014 to October 2015, 35 patients with benign thyroid nodules were treated with RFA under local anesthesia. The 18G RF needle was placed in the center of thyroid tumor under the guidance of color Doppler. Day for ablation. Output power 3W, ablation time due to tumor size and composition of different, usually 3 ~ 20 min. Results All cases were successfully treated with RFA. There was no obvious complication during operation. One case had puncture site infection within 7 days after operation. Nodules were completely absorbed in 34.2% (12/35) patients at 6 months after RFA, the volume of nodules in 48.5% (17/35) was reduced by 50% or more, and the volume of nodules in 17.1% (6/35) Shrink 25% to 50%. Conclusion The ultrasound-guided treatment of benign thyroid nodules with short-term RFA marked effect, clinical curative effect is a worthy of clinical promotion of surgical methods, but the long-term effect remains to be followed up and evaluation.
