目的对高血压患者应用组织多普勒超声和实时三维超声研究分析LVEF与MAD的相关性,进一步了解高血压患者心脏结构及功能。方法选择2014年2—12月收治的高血压伴左室肥厚患者23例作为观察组,同时选择健康体检者30例作为对照组。采用组织多普勒超声对心切面的心尖四腔进行存图,利用组织二尖瓣环位移(tissue mitral annular displacement,TMAD)软件测量运动曲线。应用实时三维超声心动图测得左室射血分数(left ventricular ejection fraction,LVEF),左室最大充盈速率(peak filling rate,PFR)。记录并比较两组心率、LVEF、左室舒张末期容积(left ventricular end-diastolic volume,LVEDV)、左室收缩末期容积(left ventricular end-systolic volume,LVESV)、PFR、二尖瓣环中点处最大位移(mitral annular displacement,MAD)水平。计量资料采用t检验,LVEF与MAD的关系采用Pearson相关分析,P<0.05为差异有统计学意义。结果观察组PFR、MAD分别为(142.28±30.03)ml/s、(8.81±1.84)mm,低于对照组的(177.24±47.29)ml/s、(15.33±1.61)mm,差异均有统计学意义(均P<0.05)。LVEF与MAD呈正相关关系。结论应用组织多普勒超声和实时三维超声可以准确、方便地评价左室舒张和收缩功能,进而证实LVEF与MAD具有良好的相关性。
Objective To study the relationship between LVEF and MAD in patients with hypertension by using tissue Doppler ultrasound and real-time three-dimensional ultrasound to further understand the cardiac structure and function in hypertensive patients. Methods Twenty-three hypertensive patients with left ventricular hypertrophy admitted from January to December 2014 were selected as observation group and 30 healthy subjects were selected as control group. Tissue Doppler echocardiography was performed on the apical four-chamber view of the coronal section. Tissue mitral annular displacement (TMAD) software was used to measure the motion curve. Left ventricular ejection fraction (LVEF) and peak filling rate (PFR) were measured by real-time three-dimensional echocardiography. The heart rate, LVEF, left ventricular end-diastolic volume (LVEDV), left ventricular end-systolic volume (LVESV), PFR, midpoint of mitral annulus were recorded and compared The maximum displacement (mitral annular displacement, MAD) level. Measurement data using t test, LVEF and MAD using Pearson correlation analysis, P <0.05 for the difference was statistically significant. Results The levels of PFR and MAD in the observation group were (142.28 ± 30.03) ml / s and (8.81 ± 1.84) mm, respectively, which were lower than those in the control group (177.24 ± 47.29) ml / s and (15.33 ± 1.61) mm respectively Significance (all P <0.05). LVEF and MAD were positively correlated. Conclusion Tissue Doppler ultrasound and real-time three-dimensional echocardiography can be used to evaluate left ventricular diastolic and systolic function accurately and conveniently, which confirms the good correlation between LVEF and MAD.
