目的 对比双胎新生儿之间脐动脉血气分析结果,分析双胎中第一胎儿与第二胎儿的新生儿脐动脉血气特点及临床意义.方法 选取2015年1月至2017年1月我院产科分娩的双胎妊娠114例相关资料进行回顾性分析,对比分析第一胎儿与第二眙儿的新生儿脐动脉血气分析中的pH、PCO2、PO2、阴离子间隙、BE水平.结果 第一胎儿的pH值要高于第二胎儿(t=3.01.P=0.003),第一胎儿的PCO2要低于第二胎儿(t=-2.41,P=0.018).第一胎儿与第二胎儿之间的pH差值与孕周存在负阳关(r=-0.289,P=0.002).结论 双胎分娩时,第二胎儿较第一胎儿易发生潜在的缺氧风险,应根据孕妇及胎儿具体情况选择最适宜的分娩时机、分娩方式,缩短分娩间隔时间,以有效减少新生儿窒息的发生.“,”Objective:To compare the results of umbilical-cord arterial blood gas analysis and analyse the characteristics and clinical significance of umbilical-cord arterial blood gas of the first and second fetus in twins.Methods:A retrospective analysis of 114 cases of twin pregnancies from January 2015 to January 2017 for obstetric delivery in Foshan Maternity & Children Health Care Hospital ()ffiliated Southern Medical University.Comparative analysis of the first fetal and second fetal umbilical arterial blood gasanalysis of the newborn's pH,PCO2,PO2,anion gap,BE level.Results:The pH value of the first fetus is higher than that of thesecond fetus (t=3.01,P=0.003),the first fetal PCO2 is lower than the second fetus (t=-2.41,P=0.018).The pH differencebetween the first fetus and the second fetus has a negative correlation with gestational week (R=-0.289,P=0.002).Conclusion:When twin births,the second fetus is more prone to latent hypoxia risk than the first fetus,it should be based on the specificcircumsta()ces of pregnant women and fetuses to choose the most appropriate timing of delivery and delivery methods,and shortenthe delivery interval in order to effectively reduce the incidence of neonatal asphyxia.
