目的:探讨老年2型糖尿病住院患者降糖药治疗期间低血糖的发生率和临床特征。方法:将397例2型糖尿病住院患者分为老年组(≥60岁)和非老年组(<60岁),入院时空腹血糖≥18.0 mmol·L-1的患者采用胰岛素治疗,<18.0 mmol·L-1的患者采用非胰岛素治疗;观察两组患者使用胰岛素和非胰岛素时低血糖的发生率及各组低血糖总发生率;分析两组患者低血糖发生时空腹血糖的降低程度、低血糖发生时的症状及低血糖好发时间等。结果:老年组患者应用胰岛素和非胰岛素各自的低血糖发生率、低血糖总发生率均明显高于非老年组患者(P<0.05、P<0.01);空腹血糖≤3.0 mmol·L-1的发生率老年组明显高于非老年组(P<0.05);低血糖的症状中老年组除颤抖的发生率与非老年组差异无统计学意义外,其他症状的发生率均明显低于非老年组(P<0.01、P<0.05);低血糖发生时间,老年组患者凌晨和晚上睡前发生率明显高于非老年组(P<0.05)。结论:老年2型糖尿病患者降糖治疗期间容易发生低血糖,而且其血糖降低程度显著,但老年2型糖尿病患者低血糖发生时的症状不明显,容易延误病情,故对老年2型糖尿病患者住院降糖药治疗期间应加强病情观察和血糖监测。
Objective: To investigate the incidence and clinical characteristics of hypoglycemia in elderly patients with type 2 diabetes inpatients with hypoglycemic agents. Methods: 397 type 2 diabetic inpatients were divided into the elderly group (≥60 years) and the non-elderly group (≤60 years). Conclusion: Hypoglycemia is prone to happen during the hypoglycemic treatment of elderly patients with type 2 diabetes mellitus, and the degree of hypoglycemia is significant. However, the symptoms of hypoglycemia in elderly patients with type 2 diabetes are not obvious and can easily be delayed. Therefore, the elderly patients with type 2 diabetes are hospitalized Hypoglycemic agents should be strengthened during treatment and blood glucose monitoring.
